Clinical Efficacy Of Huoluo Zhuyu Decoction Combined With Low Molecular Weight Heparin Sodium Injection In Prevention Of Deep Vein Thrombosis After Hip Replacement

Objective To observe the clinical effect of Huoluo Zhuyu recipe combined with low molecular weight heparin sodium injection on the prevention of deep venous thrombosis DVT after hip arthroplasty.Methods From September 2015 to December 2017,60 cases of total hip arthroplasty due to femoral neck fracture or avascular necrosis of femoral head were randomly divided into treatment group(n = 30)and control group(n = 30).The treatment group was treated by oral administration of Huoluo Zhuyu decoction and subcutaneous injection of low molecular weight heparin sodium injection.The control group was subcutaneously injected with low molecular weight heparin sodium injection.The incidence of DVT,the changes of coagulation function and the swelling of lower extremities were used to observe the incidence of DVT from the 1st day to the 14 th day after operation.Results 1.incidence of DVT: On the 14 th day after operation,there was 1 case of DVT in both groups,in which there was no DVT in the treatment group and 1 case in the control group,and 1 case was cured by taking active treatment measures.2.Examination of coagulation function: There was no significant difference in the results of APTT?PT and D-dimer between the two groups on the first day after operation,T = 1.405;0.448;1.072;P = 0.165;0.656;0.289,All P > 0.05.There was a statistically significant difference in the results of APTT?PT and D-dimer between the two groups on the 14 th day after operation,T=3.004;3.999;-2.444,P = 0.004;0.000;0.018,All P<0.05.The comparison of APTT?PT and D-dimer on the 14 th day after operation and the first day after operation in the treatment group had statistical significance,T =-7.175;-9.101;9.409,P = 0.000(P<0.05).In the control group,there was a significant difference between the APTT?PT and D-dimer on the 14 th day after operation and the first day after operation,T =6.582;-7.316;7.995,P = 0.000(P<0.05).3.Swelling of lower limbs: There was no significant difference in the swelling of lower extremities between the two groups on the first day after operation,?2=0.082,P=0.774(P < 0.05).There was no significant difference in the swelling of lower extremities between the two groups on the 14 th day after operation,?2=4.706,P=0.030(P<0.05).There were significant differences in the number of patients with lower extremity swelling on the 14 th day after operation and the first day after operation in the treatment group,?2=25.909,P=0.000(P<0.05).The negative rate of swelling was 95.24% in the treatment group.In the control group,there was a significant difference between the fourteenth days after operation and the number of swelling cases in the first world after the operation,?2=13.067,P=0.000(P<0.05).The swelling-negative rate of the control group was 64.64%.Conclusion 1.The combination of low molecular weight heparin sodium injection and traditional Chinese medicine Huoluo Zhuyu prescription alone can significantly reduce the incidence of lower limb DVT after total hip arthroplasty in patients with low molecular weight heparin injection alone.2.The combination of Huoluo Zhuyu decoction and low molecular weight heparin sodium injection could significantly prolong the time of APTT?PT and reduce the content of D-dimer.Thus,it can better improve the blood hypercoagulability of patients after operation.It plays an important role in preventing the formation of lower limb DVT after total hip replacement.3.The combination of Huoluo Zhuyu recipe and low molecular weight heparin sodium injection can obviously reduce the swelling of lower extremity after operation,which is more effective than low molecular weight heparin sodium injection alone,and plays an important role in improving blood microcirculation.Therefore,it can play a certain role in the prevention of postoperative lower limb DVT.
