Bone Cement Vertebroplasty Combined With Zoledronic Acid For Treatment Of Osteoporotic Vertebra Compressive Fracture

Objective: To evaluate its clinical effect by a comparative study of integrated therapy and bone cement vertebroplasty in the treatment of osteoporotic vertebra compressive fracture.Methods:From February 2012 to August 2012,a total of 43 patients with osteoporotic vertebral compression fractures were enrolled and were randomly divided into 2 groups: group A underwent bone cement vertebroplasty; group B underwent bone cement vertebroplasty combined with zoledronic acid. The group A(21patients, 28 vertebraes) included 5 males and 16 females with an average age of 66.5( range, 51-79 years old) while 22 patients(33 vertebraes)were in the group B who were 8 males and 14 females with an average age of 70.1(range,55-82 years old). The number of the PKP is 6,PVP is 15 in the group A,while PKP is 5,PVP is 17 in the group B. All patients were followed up for 1 week, 1 month and 6 months, according to the VAS score and bone density measurement results to evaluate the therapeutic effect, and to observe the operation complication and drug adverse reaction to evaluate the safety of the treatment.Results:The two groups have no statistically significant differernce in terms of gender, age and surgical approaches. At 1 week and 1 month and 6 months after therapy, the two groups, VAS score compared with that of preoperation decreased significantly, the differences were statistically significant(P<0.05). The VAS score before operation and 1 week after operation, A group were(8.00±1.00)and(3.76±0.89),B group were(8.18±1.01)and(3.68±0.95),no statistically significant difference between two groups(P>0.05). At 1 month and 6months after therapy, B group VAS score were( 2.09±0.75) and(1.23±0.43),and A group were(3.14±0.79)and(2.81±0.68),there was statistically significant difference between group A and group B(P<0.05). The BMD in group A and group B showed no significant difference before the therapy(P>0.05). Comparison of the BMD before and after treatment, the BMD were(0.739±0.058)g/cm2 6 months after therapy and(0.728±0.56)g/cm2 before treatment in group A, no statistically significant difference between two measurement results(P>0.05), the BMD were(0.810±0.030)g/cm2 6 months after therapy was improved obviously compared with the measurement result(0.716±0.061)g/cm2 before treatment in group B, there was statistically significant difference(P<0.05). The BMD after 6 months, the measurement result of group B was significantly higher than that of group A, the differences were statistically significant(P<0.05).Conclusion:PKP and PVP are effective in the treatment of Osteoporosis Vetebral compression fracture.The curative effect of bone cement vertebroplasty combined with zoledronic acid to treat osteoporotic vertebra compressive fracture was affirmative, could quickly relieve back pain, effectively improve bone density and reduce refracture risk.
