Objective: In order to find out the connection between level of TGF-β1, 2, 3 and COPD patients’ lung function and sputum cytology, the research has been adopted the strategy of ① measuring induced sputum concentration of AECOPD, COPD patients and health control group, ②separating macrophages sacks in sputum, ③observing Smad2/3 expression under Immunofluorescent Staining. To sum up, the function and clinical significance of TGF-βand AMs for COPD patients could be stated as follows. Methods:Measure the FEV1% the lung function and make induced sputum inspection to three groups of samples. They are 14 COPD acute patients in Respiratory Department of Sichuan medical University attaching hopital, 15 COPD stable patients in clinic or hospital and 8 healthy volunteers. First the level of TGF-β1、TGF-β2、TGF-β3 in their induced sputum are measured; then the sputum sample will be calculated in terms of cytology, AMs will be extracted and isolated with adherence method and its Smad2/3 will be observed by marking with Immunofluorescent Staining. After that there will be biopsy of airway mucosa respectively from 8 COPD stable subjects and 4 uninjured side lungs in the controlling group, using the technology of bronchoscope. In the further step, collagen and mucus would be known by observing the massoned and PAS sample sacks。And finally all the observation results will be put into SPSS statistics software to work out the connection between level of TGF-β1, 2, 3 and COPD patients’ lung function and sputum cytology.Result:1.Almost all the participants of the three groups could endure induced sputum inspection with only one subject occurred with aggravating dyspnea. The subject was relieved soon by inhaling 200 ug Salbutamol and then finished the inspection successfully.2.Induced sputum cells number of COPDpatients is bigger than the controlling health group but smaller than the AECOPD group(P<0.05). Neutrophile granulocyte is generally observed in the sputum of COPD groups and its number is rising when the phase transmitted from stable to acute. While in health group macrophages and lymphocyte are taking the priority(P<0.05). Rate of eosinophilic granulocyte is lower in AECOPD than in other 2 groups whereas there is no clear gap between the latter two(P>0.05). All subject are alike in the number of epithelial cells(P>0.05)3.AMs number in AECOPD is 1.12±0.16)×106/L while(0.84±0.20)×106/L in COPD. It turns out to be negative correlation between AMs cell population and FEV1%(r=-0.748,P<0.05) by analyzing AMs and FEV1% of the patients with Pearson Method.4.AMs are negative correlated with content level of TGF-β1、TGF-β2 among the AECOPD and COPD stable phase patients in their induced sputum experiments. But there is no such sign among health people. It is indicated that there is no linear correlation in terms of AMs and TGF-β3 between COPD patients and health people.5.Index TGF-β1、TGF-β2、TGF-β3 of COPD patients are apparently higher than that of healthy people. And the same index is found higher in AECOPD than in COPD stable phase group.(P<0.05)6.TGF-β1、TGF-β2 is negative correlated with FEV1% while TGF-β3 is positive correlated.The r value of TGF-β1、TGF-β2、TGF-β3 is-0.560,-0.640, 0.613 respectively in AECOPD, while-0.689,-0.622,0.590 in COPD(P<0.05).7. In healthy people’s macrophages, Smad2/3 is mainly expressed weak in cytoplasm while in COPD stable group is much stronger expressed in cytoplasm and cell nucleus. AECOPD patients have a great similarity with COPD stable group except that its expression strength is even stronger.8.collagen scores 5.00±1.069 in COPD group compared with 1.75±0.71 in control group.9. COPD patient induced sputum TGF-β1, TGF-β2, expression and alveolar macrophages Smad2/3 mean fluorescence intensity value TGF-β3 was positively correlated, r values were 0.620,0.616,0.488, P<0.05.Conclusion: 1.COPD induced sputum in patients with different stages of TGF-β1,TGF-β2,TGF-β3 content, and there is a certain correlation with lung function, they may play an important role in airway remodeling in COPD patients, by Determination of induced sputum TGF-β1, TGF-β2, levels of TGF-β3 estimates of patient lung function. 2.COPD different pathological stages of induced sputum alveolar macrophages Smad2/3 expression is different, and the expression was induced sputum and TGF-β1, TGF-β2, TGF-β3 levels of correlation, suggesting that TGF-β1,2 3- Smad2-3 expression enhance is one of the important reasons for the development of COPD alveolar macrophages may be involved.
