Objective:To investigate the relationship of polymorphisms of CD36 gene rs17154181, rs 1761667 and macrovascular complications in type 2 diabetes mellitus, using analysis of variance to investigate biochemical difference between three groups (including type 2 diabetes without macrovascular complications group, type 2 diabetic patients with subclinical atherosclerosis group, type 2 diabetes with macroangiopathy group).Logistic regression model was to analysis the risk factors contributing to f type 2 diabetes mellitus and large vascular lesion.Methods:112 cases of T2DM and 65 non diabetic controls from Guangxi Zhuang Autonomous Region long-term unrelated individuals.112 cases in the case group, according to 1999 WHO recommended standards of diagnosis and classification of diabetes mellitus, selected in December 2012 to June 2014 in the first affiliated hospital of Guangxi Medical University Endocrinology and metabolism section inpatients with type 2 diabetes, male 70 cases, female 42 cases, age (57.13±9.80) years old. According to the large vascular disease of the degree is divided into three sub groups, group A:type 2 diabetes without macrovascular complications group 39 cases, male 26 cases, female 13 cases, age (50.15± 8) years old; group B:subclinical atherosclerosis in type 2 diabetes mellitus group 46 cases, male 27 cases, female in 19 cases, age (60.91±8.49) years old; group C:type 2 diabetes with macroangiopathy group 27 cases, male 17 cases, female 10 cases, age (60.74± 8.95) years old. Normal control group: 65 cases selected from our hospital physical examination center health examination population over the same period, ages (52.68± 10.753) years old, male 39 cases, female 26 cases, the fasting and postprandial blood glucose excluding diabetes. Using polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP) genotype analysis method to detect the genotype of CD36 gene rs 1761667, rs17154181, height, weight, blood pressure specified by the same person measurement, body mass index (BMI)= body weight (Kg)/ height (m2). Using automatic biochemical determination to analyse serum total cholesterol, triglyceride, low density lipoprotein, high density lipoprotein, fasting blood glucose, postprandial blood glucose, glycosylated hemoglobin. To analyse the genotype and allele frequency differences between control group and the case group, case group with large lesions and with subclinical atherosclerosis and without macroangiopathy group, using analysis of variance to investigate biochemical difference between three groups (including type 2 diabetes without macrovascular complications group, type 2 diabetic patients with subclinical atherosclerosis group, type 2 diabetes with macroangiopathy group).Logistic regression model was to analysis the risk factors contributing to f type 2 diabetes mellitus and large vascular lesion.Results:1. According to the 1999 WHO recommended diabetes diagnosis and typing standard, in T2DM cases,CD36 gene polymorphism of rs17154181 of three kinds genotype of AA, AG, GG population respectively is 21 people,53 people and 38 people, respectively in this group accounted for 18.75%,47.32% and 33.93%; in the control group, AA, AG, GG three kinds of genotype number respectively is 19 people,25 people and 21 people, respectively in this group accounted for 29.23%,38.46%,32.31, no significant differences in the genotype frequency between the two groups (P=0.251). According to the 1999 WHO recommended diabetes diagnosis and typing standard, type 2 diabetes mellitus cases of CD36 gene polymorphisms of rs 1761667 AA, AG, GG three kinds of genotype population respectively is 22 people,49 people and 41 people, respectively in this group accounted for 19.64%,43.75% and 80.36%. In the control group the sites of AA, AG, GG three kinds of genotype population respectively is 7 people,27 people and 31 people, respectively in this group accounted for 10.77%,41.54% and 47.69%, the difference was not statistically significant for each genotype frequency between the two groups (P=0.195).2. Comparison of three groups of clinical data of,Type 2 diabetes without macrovascular complications group, T2DM with subclinical atherosclerosis group and type 2 diabetes with macroangiopathy group:age is a risk factor for type 2 diabetes macroangiopathy. Cases were grouped according to the genotype of CD36 gene polymorphisms rs17154181 and rs 1761667, age, body mass index, fasting blood glucose, diastolic blood pressure, total cholesterol, triglyceride, low density lipoprotein, high density lipoprotein, no significant difference is finded.In rs17154181AG genotype group, found that systolic blood pressure than AA genotype group systolic blood pressure is high, and the difference in rs 1761667 group were not found.3. Logistic regression analysis on risk factors of type 2 diabetes and its macroangiopathy, found that age, history of hypertension were significant correlation diabetic macroangiopathy, two polymorphisms genotypes were not in the equation.Conclusions:1. CD36 gene polymorphisms rs17154181 and rs1761667 genotype distribution in type 2 diabetes group and normal control group had no statistical difference.;in type 2 diabetes mellitus controls, type 2 diabetic patients without macrovascular complications group and type 2 diabetic patients with subclinical atherosclerosis group and type 2 diabetes with macroangiopathy group the genotype of rs17154187、rs 1761667 distribution have no significant difference.
