Jiawei Jianpi Yishen Decoction On Clinical Study Of OPN In Urine Of Renal Calcium Oxalate Stone Patients After Operation

BackgrundUrinary osteopontin (OPN) is one of the most important matrix protein in forming of renal calcium oxalate stones, which can regulate mineralized tissues,inhibit the nucleation, growth and aggregation of calcium oxalate crystallization, were the main pathogenesis of urolithiasis. Thus, through observing the clinical effect of Jiawei Jianpi Yishen Decoction on urinary OPN in calcium oxalate nephrolithiasis patients after Percutaneous Nephrolithotomy (PCNL) or Ureteroscopy (URL), we try to make a beneficial exploration for Traditional Chinese medicine prevention of urolithiasis.ObjectiveThe purpose of our study is to observe the effect of Jiawei Jianpi Yishen Decoction on urinary osteopontin (OPN) in calcium oxalate nephrolithiasis patients after PCNL or URL; And to clarify the mechanism of Jiawei Jianpi Yishen Decoction on the prevention of calcium oxalate kidney stones.MethodsAdopted 70 calcium oxalate nephrolithiasis patients and was divided into 35 control groups (G1) and 35 experimental control groups (G2) at random. The experimental control groups (G1) took Jiawei Jianpi Yishen Decoction every other day while the other group (G2) took Potassium Sodium Hydrogen Citrate Granules three times a day. Double antibody sandwish enzyme-linked immunosorbent assay (ELISA) was determinated of urinary OPN content. Detector measured by automatic biochemical content of urinary calcium and oxalate. Before and after treatment in 2 weeks and 4 weeks, each patient’s TCM symptome score index table was recorded.ResultsConclusion of comparability:There was no significant difference of age, urinary OPN, urinary calcium; urinary oxalate, TCM syndrome score in Gl and G2(P>0.05) before treatment.Conclusion of the effects analysis:After one month of therapy, there are remarkable rise of the level of urniary OPN both in G1 and G2. (P< 0.05). Compared with G2, G1 had a higher increasing rate of urniary OPN after PCNL or URL, with a statistically significant difference (P<0.05).The level of urinary calcium and urinary oxalate of G1 and G2 after one month of therapy showed a significant reduction(P<0.05). Contrasted with G2, there is singnificant difference in G1(P<0.05).After 1 month of treatment, the total efficiency of G1 group reaches 80.00%, while attaines 57.15% in G2.The total effective rate of TCM symptome score decreases from pre-treatment (12.49±2.81) to (9.31 ±2.48) in G1 group, while the TCM symptome score decreases from (12.66±3.15) to (12.26±2.96) in G2 group. Therefore, G1 group has a statistically difference(P<0.05).ConclusionThe results show that Jiawei Jianpi Yishen Decoction can effectively restrain the formationn of the calcium oxalate stones through increasing the level of urinary OPN, whlie reducing the level of urinary calcium and oxalate. Moreover, it can significantly reduce the patients’TCM symptoms integral as well as improve the life quality for postoperative patients after PCNL or URL, which demonstrate a effective prevention in the calcium oxalate nephrolithiasis.
