Objective: Wheezing is the most common respiratory symptoms of infants. Among the infants who has respiratory tract infections in winter,70% suffers wheezing,and has a rising trend year by year, some of them develop to bronchial asthma. Bronchial asthma is a chronic airway inflammatory disease which is composed of variety of cells and cellular components, Clinical characterized by repeated attacts of wheezing, shortness of breath,chest tightness, cough and other symptoms. Bronchial asthma risks our healthy,if controlled unsatisfactory, affacts children from normal life, parents work, and brings burden to society.So as soon as possible to discover infants who has asthma, and cure them in early time, has significant meaning to improve the diagnosis. This research detects the level of serum Interleukin-4,Interleukin-37 and Interferon-γ and compare them, aims to discuss the role of serum Interleukin-4,Interleukin-37 and Interferon-γ in infants with asthmatic diseases, analyze the association with the pathogenesis of brochial asthma,and evaluates prognosis of children, and make a contribute to early diagnosis and cure for asthma.Methods: We selected 42 infants who has asthmatic disease from inpatients in pediatric ward of NO.2 hospital of hebei medical university during January 2014 to January 2015,including 20 infants who wheeze the first time and 22 infants wheeze two or more times; they had both been separated into the group before cure, and the group after cure. They are all under the age of three, had not been using glucocorticoid for 2 weeks, get the agreement of their parents, exclude those who has congenital heart disease, congenital bronchial pulmonary hypoplasia, bronchial foreign body, congenital malformation of the airway, and with other immune disease. Select children under the age of 3 of normal physical examination cases in our pediatric medical outpatient service as control group. The age and sex constituent ratio had no obvious difference among all groups in this research. Collect venous blood in all groups, after centrifuged, serum specinens were stored in-70 ℃ refrigerator. The level of serum Interleukin-4,Interleukin-37 and Interferon-γ in all groups was examined with sandwich ELISA methods. The data is treated with statistics software SPSS13.0 for windows. Statistical analysis was performed using Chi-square test, one-way ANOVA, t test. P-value<0.05 was considered to be significant.Results:1 Level of serum IL-37:First time wheezing group before cure: 42.41±3.75ng/L;First time wheezing group after cure:50.97±3.51ng/L;Two or more times wheezing group before cure:35.42±3.02ng/L;Two or more times wheezing group after cure : 40.20 ± 2.82ng/L; Control group : 51.64 ±4.20ng/L;Statistic data above,we conclude:(1)The serum level of IL-37 in first time wheezing group is higher than that in more times wheezing group,The difference is statistically significant;(2)The serum level of IL-37 in more times wheezing group before cure is lower than first time wheezing group, both lower than control group. The difference is statistically significant;(3)After cure, The serum level of IL-37 in more times wheezing group is lower than that in first time wheezing group and control group,but the level in first time wheezing group and control group has no significant difference.2 Level of serum IL-4:First time wheezing group before cure: 215.27±21.64ng/L;First time wheezing group after cure:192.66±14.70ng/L;Two or more times wheezing group before cure:223.19±13.59ng/L;Two or more times wheezing group after cure:205.93±13.38ng/L ng/L;Control group:185.45±13.37ng/L;Statistic data above,we conclude:(1)The serum level of IL-4in first time wheezing group is lower than that in more times wheezing group,The difference is statistically significant;(2)The serum level of IL-4 in more times wheezing group before cure is higher than first time wheezing group,both higher than control group. The difference is statistically significant;(3)After cure, The serum level of IL-37 in more times wheezing group is higher than that in first time wheezing group and control group,but the level in first time wheezing group and control group has no significant differences.3 Level of serum IFN-γ:First time wheezing group before cure: 623.44±27.75ng/L;First time wheezing group after cure:741.23±27.86ng/L;Two or more times wheezing group before cure:583.85±28.7ng/L;Two or more times wheezing group after cure: 634.81±28.17ng/L;Control group:740.54±40.97ng/L;Statistic data above,we conclude:(1)The serum level of IFN-γ in first time wheezing group is higher than that in more times wheezing group,The difference is statistically significant;(2)The serum level of IFN-γ in more times wheezing group before cure is lower than first time wheezing group,both lower than control group. The difference is statistically significant;(3)After cure, The serum level of IFN-γ in more times wheezing group is lower than that in first time wheezing group and control group,but the level in first time wheezing group and control group has no significant difference.4 The serum IL-37 level was negtively correlated with the serum IL-4 level, and positively correlated with the serum IFN-γ level in children with asthmatic disease.Conclusion:1 The change of serum level of IL-37,IL-4 and IFN-γ prompts that they invole in infantal asthmatic disease pathogenesy.2 Serum level of IL-37,IL-4 and IFN-γ in two or more times wheezing group after cure is still different with control group,promt that these children had more possibility to develop into asthma, and its immunological pathogenesis may be similar to the mechanism of bronchial asthma.3 Detection of serum level of IL-37,IL-4 and IFN-γ in the wheezing infants can be used as a basis of early diagnosis and early intervention for children who will have asthma.
