Objective To investigate the clinical characteristics of primary ANCA associated small vasculitis (AAV) in guangxi han nationality and the relationship between TLR4 gene promoter region—1837A/G polymorphism and AAV.Methods The clinical data of 110 AAV were retrospectively analyzed, and TLR4-1837A/G polymorphism was analyzed by polymerase chain restricted fragments length polymorphism in 110 patients and 101 controls. Clinical and pathologic data of the patients with different genotype were also compared.Results The involvement organ of 110 cases with AAV:95 cases of kidney (86.4%),75 cases of lung (68.2%),45 cases of digestive tract(40.9%),25 cases of joint (22.7%),23 cases of skin mucous membrane (20.9%),15 cases of cardiovascular (13.6%),11 cases of the nervous system (10.0%),8 cases of eyes (7.3%).102 ANCA cases(92.7%) of the patients were positive,90 cases of hemoglobin were dropped (81.8%),100 cases of CRP and 105 cases of ESR were elevated. The frequencies of TLR4 -1837A/G genotypes in 110 AAV patients were AA:53.64%,AG:40.00%, GG:6.36%, respectively; and frequencies of alleles A and G were 73.64%、26.36%. While in controls,genotype frequencies were AA:50.50%,AG:39.60%, GG:9.90%,respectively; frequencies of alleles A and G were 70.30%、29.70%; No significant difference was found in either genotype distributions or allele frequencies between the patients and the controls(P>0.05). Significant reduction in the incidence of Hematuria and serum C reactive protein、erythrocyte sedimentation rate was found in AG genotype than the other two genotypes, as the Hemoglobin values of AG genotype were significantly higher than the others (P<0.05).Conclusion AAV can infringe multiple organs, hemoglobin value usually decrease, ANCA detection can help diagnose AAV.In the Han nationality in Guangxi,the polymorphism of TLR4-1837 A/G may be associated with AAV in patients with hematuria, hemoglobin, disease activity index, but the genetic susceptibility to AAV may be not related.
