BackgroundThe First-line treatment of advanced colorectal cancer has becomed well-developed, however, the effect of chemotherapy has reached its own bottleneck, and the accumulation of chemotherapy side effects, limited targeted population and high cost make it difficult for its widely use in maintenance treatment of advanced colorectal cancer. On the other hand, traditional Chinese medicine(TCM) has the advantege to some extent in relieve discomforts, improving quality of life and finally make it true to survive with tumor. Therefore, more and more studies began to explore the role and prospect of TCM in maitenance therapy of advanced colorectal cancer. At the same time, the application of TCM will definitely be a useful and benificial supplement in the comprehensive treatment of advanced colorectal cancer.PurposeTo include the patients who at least recieved second-line chemothrapy and achieve disease control after chemotherapy.Comparatively analysis the progression-free survival(PFS),the weight, quality of life (including KPS scores and the symdrome scores) and adverse drug reactions in two groups of patients. Therefore it is benificial to prove the improtance and exact efficacy of TCM in maitenance therapy of advanced colorectal cancer.MethodBased on inclusion and exclusion criteria, divide the suitable patients into 2 groups, including Xeloda group Of 23 cases, TCM group of 22 cases. The main outcome was PFS, weight, symdrome scores, Karnofsky score and adverse drug reactions, which contribute to the evaluation of Clinical efficacy.ResultThe study included 45 qualified cases, including Xeloda group Of 23 cases, TCM group of 22 cases. In comparison of baseline, the differences between two groups were not Statistical significance, so the two groups were comparable. The results indicate that:The median PFS of TCM group was 5.3 months, while the median PFS of Xeloda group 4.9 months. There were no significant differences between the two groups (P> 005), while the Karnofsky score had obvious effect on patients’PFS; In terms of weight, symdrome and Karnofsky score, the difference before and after treatment were significantly different (P<0.05) in both groups; and the TCM group recieved better improvement than Xeloda group in weight and symdrome score, the differences of which were also statistically significant (P<0.05). Serious Malpractice did not occur during treatment in two groups. Less hematological, liver and kidney toxicity had showed in TCM group, and the difference had stastically significance(P<0.05). In the two aspcets of diarrhea and hand-foot syndrome, which are common side effects of Xeloda, TCM group showed lower incidents than the Xeloda group, and the difference was also statistically significant (P<0.05).ConclusionsThe prescription of "Remove Blood Stasis and Relieve Toxicity" can extend the patients’PFS, which is not inferior to Xeloda. Meanwhile, the prescription can obviously relieve discomforts, upgrade Karnofsky scores and symdrome scores, and gain weight, which result in the improvement of quality of life in patients of advanced colorectal cancer. Therefore, The prescription of "Remove Blood Stasis and Relieve Toxicity" is applicable to the most patients in maintenance treatment. What’s more, its safety, efficacy and convienience are worthy of further research and exploration.
