| 1. Objective To observe the clinical therapeutic effect of Yiqi Jianpi Granule in the treatment of the spleen and stomach weakness type of chronic atrophic gastritis, then make an objective evaluationand and investigate the possible mechanism,in order to provide a basis for further clinical and experimental research.2. Method There are two parts in the article, including journals reviewed and clinical research.First Part:There are two papers in the first part, the first chapter summarizes the replenishing qi to invigorate the spleen of chronic atrophic gastritis in TCM research situation, effective method the Jianpi method treatment of CAG in the role of TCM;The second states the study of CAG in the field of modern medical research,elaborated the possible etiology and pathogenesis, treatment method.Second Part:Choose the patients matched with the inclusion criteria were allocated randomly into two groups:The course of treatment was 12 weeks. Recording the clinical symptoms,gastroscope change and pathology effect, GAS levels,PG levels,under the efficacy and long-term situation change of the patients in two groups before and after treatment.In the two group as a case of Helicobacter pylori infection, at the same time take drug eradication of Helicobacter pylori, period of treatment is 10 days, review the results of Helicobacter pylori infection after 1 months, and recording.3. Results3.1 Clinical symptoms:The clinical symptoms of the two groups were improved after the treatment(P<0.05).The total effective rate of treatment group is higher than that of control group(P<0.05).Except gastric fullness, anorexia, belching.Treatment group improement clinic symptoms rates than the control group and clinical markedlyfaster.The comparison is of evident statistics significance(P<0.01 or P<0.05).3.2 Endoscopy: The treatment group endoscopic gastric mucosal improve efficiency rate was 90%, 62.07% in the control group, there was difference between two groups(P<0.05).3.3 Histopathology:On the gastric mucosa atrophy and improve the curative effect, the effective rate of treatment group was 86.67%, 62.07% in the control group, there was difference between two groups(P<0.05);Improvement effect on gastric mucosal intestinal, the effective rate of treatment group was 86.96%, 57.14% in the control group, the two groups had difference(P<0.05);On gastric mucosal dysplasia curative effect, the effective rate of treatment group was 63.64%, 55.56% in the control group,the difference between the two groups had no statistical significance(P>0.05).3.4 Eradication of H.pylori infection: the Hp seroconversion rates of two groups after treatment were 90.48% and 73.68%, there were no statistical differences in differences(P>0.05).3.5 G17 levels:G17 levels in the two groups after treatment than before treatment were increased, with significant difference(P<0.05).And the G17 levels of treatment group increased more obvious than the control group, the difference was statistically significant(P<0.05).3.6 PG levels:Treatment group after treatment PGI:96.39±36.12, PGII:15.27±13.71,PGI/PGII:6.35 ± 2.94; Control group PGI:85.17 ± 21.02, PGII:19.75 ± 12.53,PGI/PGII:5.53 ± 2.16. By the test, there was significant difference between the two groups of curative effect(P<0.05), the treatment group superior to the control group.3.7 The long-term curative effect observation: follow-up twelfth week period.the total effective rates of improving the TCM symptom of treatment group was 90%, control group was 58.62%, two groups had significant difference(P<0.05). Comparison of the total effective rates of improving the TCM symptom the end of treatment and follow-up of 12 weeks,there was no statistical difference between the two groups(P>0.05).4. Conclusion4.1 Yiqi Jianpi Granule can improve the clinical symptoms of patients.Except gastric fullness, anorexia, belching.Treatment group improement clinic symptoms rates than the control group and clinical markedly faster.4.2 Yiqi Jianpi Granules can improve the situation of gastric mucosa;The treatment outcome is better than weifuchun.4.3 Yiqi Jianpi Granule in improving intestinal metaplasia,gastric mucosa atrophy is better than Weifuchun;On gastric mucosal dysplasia curative effect was equal between Yiqi Jianpi Granuleand weifuchun tablet.4.4 Compared with Weifuchun,Yiqi Jianpi Granule did not improve the negative conversion rate of H.pylori infection.4.5 Yiqi Jianpi Granules can promote the secretion of PG, GAS, better than weifuchun.4.6 Long term therapeutic effect of Jianpi Granule on CAG significantly is better than weifuchun tablet. |
