[Objective]Iodine, an essential component of the hormones produced by the thyroid gland, is widely but unevenly distributed in the earth senvironment. Great difference exists in the iodine nutritional status of populations residing in different region.Inl996,China began to implement the universal salt iodization policy, which has basically eliminated the iodined efficiency disorders nationwide; however, it also caused the changes in the spectra of other thyroid diseases including iodine-induced hyperthyroidism, autoimmune thyroid disease, and papillary thyroid carcinoma. Individualized iodine nutritional status assessment for the populations, particularly thoese with thyroid diseases, will be beneficial. To investigate the relationship between common types of thyroid diseases (thyroid papillary carcinoma, nodular goiter, thyroid follicular tumor and hashimoto thyroiditis) in Liaocheng area and the thyroid’s iodine content, urine iodine, and analyze the association between urinary iodine, thyroid’s iodine content and thyroid function, respectively.[Methods]Surgical speciments of thyroid from 108 patients suffered from thyroid disease (containing 64 benign tumors and 44 mallignant tumors) which were hospitalized from Januarry 1st,2011 to august 30th,,2012 were retrospectively analyzed. All speciments were confirmed by postoperative pathological diagnosis through conventional paraffin sections.64 pieces of specimens from benign tumors (containing 45 of nodular goiter,8 of Hashimoto thyroiditis,11 of follicular tunor) and 44 pieces of specimens from papillary thyroid carcinoma were involved. All patients were required to have taken samples of urine in the morning into the special urine cup equipped with urine iodine determination kit, and fasting 8 hours was necessary. Samples of urine were determinate and recorded within 2 hours. Serum samples of 108 patients were collected to have been measured the thyroid function parameters containing free triodothyronine,free four triodothyronine, thyroid stimulating hormone,specimens of thyroid lesions taken intraoperative were placed in 80 refrigerator for preservation. Ioidine content of thyroid tissue was determinated uniformly after all specimens were collected. Examine the levels of tissue iodine content, urinary iodine, serum concentrations of free triiodothyronine (FT3),free thyroxin (FT4) and thyroid-stimulating hormone (TSH) in four types of thyroid diseases including thyroid papillary carcinoma, nodular goiter, thyroid follicular tumor and hashimoto thyroiditis. The data were analyzed by SPSS 20.0.[Results]Mean value of iodine content of thyroid tissue of thyroid tissue of benign group was (154.92±77.77) ug/L and of malignant group was (296.48±82.28) ug/L, with P=0.001. Mean value of iodine content of urine of malignant group was significantly higher than that in benign group, with P=0.002. It was explained that, iodine was involved in pathogenesis of thyroid disease. Iodine content of thyroid tissue and urine have obvious correlation and the two have reference value in differentiation of benign and mmalignant disease.Iodine content of papillary thyroid carcinoma tissue was significantly higher than in nodular goiter group, Hashimoto thyroiditis group and follicular tumor group, withP=0.001. Among benign tumor groups, iodine content of tissue between nodular goiter group and follicular tumor group showed significant difference and no statistical difference was found between nodular goiter group and Hashimoto thyroiditis group or between follicular tumor group and Hashimoto thyroiditis group, with P=0.132,0.473 respectively.Mean value of iodine content of iodine content of urine of papillary thyroid carcinoma group was obviously higher than that in nodular goiter group, Hashimoto thyroiditis group and follicular tumor group, with P=0.002. Howeve, no statistical difference was found among benign group.Level of thyroid function parameters among different pathological types showed no statistical difference,with P>0.05. Interms of different pathological types of thyroid, no significant correlation was found between iodine content of tissue, urine and thyroid function parameters.[Conclusions]In Liaocheng area, the significant difference of the thyroid’s iodine content and urine iodine between malignant thyroid disease and benign thyroid disease shows that iodine plays some part in the pathogenesis of thyroid malignant disease. The consistency between urine iodine and thyroid’s iodine content indicates that the urinary iodine examination could predict the thyroid tissue iodine content. This particular indicator could be useful for screening the types of thyroid disease, and would be helpful for investigating iodine nutrition status of individual person.
