Clinical Research On The Combination Of Shujinhuoluoyin And Western Conwentional Medicine To Treat Diabetic Lower Vascular Lesions

ObjectiveNowadays,diabetics with vascular disease are increasing steadily. Diabetics are likely to suffer from peripheral Arterial Disease than ordinary people and it is common for diabetics to be affected by Peripheral Arterial Occulusive Disease and vascular calcification.Diabetics with Lower Extremity Arteries Disease have a risk of Lower limb amputation 15 to 40 times as high as non-diabetic patients. Although treated by western medicine, some patients can not avoid getting amputation.In order to improve the treatment effect and reduce the amputation, we have tried the treatment of combination of Chinese traditional and western medicine on 109 diabetics with Peripheral Arterial Disease, whose results is quite satisfying. on one hand, we used the basic therapy, that is, use Insulin to reduce blood sugar and vasodilators to increase local blood supply. On the other hand, we added herbal decoction with the function of promoting blood circulation and removing blood stasis to the basic therapy. It is a kind of drink named Shuijinhuoluoyin to free the network vessels. Also, we carefully observed the curative effect, according to the changes of Clinical symptoms, color Doppler examination results and laboratory blood testing. Compared with conventional treatment, there is evidence that the combination of Chinese and western medicine therapy by using herbal decoction and basic treatment of western medicine is clearly superior than the simple western medicine treatment. We have provided a new effective method to treat the diabetic lower extremity vascular disease on clinical treatment.MethodsWe chose 109 diabetic patients with lower extremity vascular disease in our hospital randomly and divided them into two groups. For the control group of 50 patients, we gave them conventional therapy including foundation treatment, the dilation of blood vessels and antiplatelet therapy. For the therapy group of 59 patients, we added the herbal decoction, houxuetongluoyin in our treatment to the conventional therapy.1.1 Conventional therapy(1) Insulin to control blood sugar(2) Proper diet and total calorie intake control.s1.2 The dilation of blood vessels and antiplatelet therapy:Physiological saline 250 mlqd with O2 agred 80mg;Infusion of Alprostadil 10 ugqd, oral aspirin 50 mg per day.1.3 Differentitation of traditional Chinese medicine;Shuijinhuoluoyin;Radix cyathulae 15 g, Red sage root 30 g, Chinese angelica Ligusticum Wallichii 12 g, Taoren 15 g,Leech 15 g,Pheretima 20 g, Lulutong 15 g, Licorice root 12 g, decocted in water for oral dose and one dose a day.Patients who are weak: add astragalus mongholicus 40 g Patients who have heat toxin: add Honeysuckle 40 g Tu fuling 50 g.Both of the two groups took 10 d for one therapealtic course and have a continuous treatment of 2 courses.Criterion of therapeutical effect:(1)The increase in walking distance(2)The increase in the double lower artery segments pipe diameter and blood flow volume.(3) The increase in ankle brachial index, and dorsalis pedis dulse index.(4) Significant increase in average dosails pedis artery blood flow velocity.(5) Sharp decline in dorsails pedis artery resistance index, the whole-blood viscosity and plasmatic viscosity and erythrocyte aggregation index.(6) The curative effect of liver and kidney function safety. We have determined that the improvement of curative effect has statistical significance after statistics processing. Result: The control group are improved in walking distance and have an obvious advantage in the double lower limb artery segments pipe diameter, blood flow volume, ankle brachial index, dorsails pedis artery resistance index, dorsaits pedis pulse index, average dorsails pedis artery blood flow velocity and atteration in hemorheology.ResultsThe two groups have statisticial significance in comparison before and after the treatment(p<0.05). The therapy group have a better result than the control group after the treatment9p<0.05) they have a clear improvement in all aspects; walking distance,double lower limb artery segments pipe diameter blood flow volume, dorsalis pedis pulse index and average dosails pedis artery blood flow velocity. And the therapy group has a lower dorsails pedis artery resistance index and hemorrheology index than the control group. The two groups have no significant differences in liver and kidney function before and after the treatment.ConclusionThe combination of shujinhuoluoyin and western conventional medicine is obviously superior than only western conventional medicine in treating diabetic lower vascular lesions.
