Objective To investigate the protection of jiangyabao blue tablet on traditional Chinese medicine against target organ damage in spontaneously hypertensive rats(SHR), primarily explore the mechanism of prevention and reversing left ventricular hypertrophy and renal damage for clinical application.Method SHR were assigned randomly into 3 groups, 10 per set: the positive control group, tblank control, Wistar-kyoto(WKY)-rats were used as negative control group;After gavafor 8 weeks, by using noninvasive sphygmomanometer to measure last arterial blopressure,left ventricular posterior wall thickness(LVPWT), interventricular septthickness(IVST), and left ventricular end-systolic diameter(LVESD) were measured ultrasound, the data were used to calculate the left ventricular fractional shortening(LVFSleft ventricular end-systolic volume(IVESV), left ventricular end-diastolic volume, IVEDVejection fraction(LVEF) and stroke volume(SV); After anesthesiaed, weighed heart and leventricular and kidney tissue from rats to calculate cardiac index(CI),left ventriculindex(LVMI)and renal index(RI); Enzyme linked immunosorbent assay(ELISA) detectthe expression of the left ventricular and kidney tissue of angiotensin(1-7) and angiotensconverting enzyme 2(Angiotensin Converting Enzyme 2, ACE2);Mas protein expressiwas detected in rat left ventricular using Western Blot.Result 1. Effect of Jiangyabao blue tablet on SHR blood pressure, heart rate Compared with the WKY group, the SBP, DBP and MAP in SHR group of rats were significantly increased(P<0.01), HR showed a rising trend, but there was no statistical difference(P>0.05); Jiangyabao blue tablet could significantly reduce the SBP, DBP, and MAP of SHR group, but the degree of dropping blood pressure was significantly lower than the enalapril; showed no significant effect on HR(P>0.05).2. Influence on SHR echocardiography Compared with the WKY group, the LVEDV, LVESV and SV in SHR group were decreased(P<0.01); though the LVST, LCPWT, LVFS and EF were increased, no statistical significan(P>0.05). The jiangyabao blue tablet can significantly reduce the FS and EF(P<0.01), but showed no significant effect on other echocardiography indicators.3. Effect of jiangyabao blue tablet on SHRCI, LVMI, RI Compared with the WKY group, SHR group CI, LVMIand RI were increased(P<0.01). Jiangyabao blue tablet can significantly reduce LVMIand RI, compared with SHR group showed significant differences(P<0.05~0.01); showed a downward trend for CI, but no statistical difference(P>0.05). Enalapril could significantly improve CI、LVMI(P<0.01), but had no significant effect about RI.4. About affect of SHR left ventricular ACE2-Ang(1-7)-Mas R Compared with the WKY group, SHR left ventricular local activity of ACE2 and content of Ang(1-7)showed a downward trend, but had no statistical difference(P>0.05); Mas R protein expression was significantly reduced(P<0.01). Jiangyabao blue tablet could significantly improve Mas R protein(P<0.05), but Jiangyabao blue tablets had no significant effect on SHR left ventricular ACE2 activity and Ang(1-7)content(P>0.05).5. About affect of SHR kidney tissue ACE2-Ang(1-7)-Mas R Compared with WKY group, local ACE2 activity in renal tissue of rats SHR group and Ang(1-7) content showed a rising trend, but there was no statistical difference(P>0.05). Jiangyabao blue tablet can significantly reduce the SHR kidney tissue ACE2 activity and Ang(1-7)content, compared with SHR group and positive control group(Enalapril) showed significant differences(P<0.01).Enalapril could decreased activity of ACE2 and content of Ang(1-7), but there was no statistical difference(P>0.05).Conclusion 1. Jiangyabao blue tablet had certain effect of dropping blood pressure, but margin of blood pressure reduction lower significantly than angiotensin-converting enzyme inhibitor enalapril; 2. Jiangyabao blue tablet could significantly improve the SHR left ventricular hypertrophy and hypertensive renal damage, in which the improvement of renal impairment was significantly better than the positive control drug enalapril,showed Jiangyabao blue tablet prevention and reversal of hypertensive target organ damage may be non-dependent blood pressure;3.The mechanism of prevention and reversing SHR left ventricular hypertrophy and renal damage was related to raise Mas receptor expression of ACE2-Ang(1-7)-Mas R axis.
