Object To determine the epidemiological situation of confirmed HIV-1 infections in Guangxi after 2008,and to understand the distribution of HIV-1 subtypes of Guangxi. To provide the basis for making strategy for prevention and control of AIDS, to provide valuable information to the prevention and control of AIDS in Guangxi, even in the whole country.Methods To make epidemiological survey on the 100、102、103 HIV-1 infections confirmed after 2008 who come from Guilin、Laibin、Qinzhou, including demographic data(age, sex, occupation, educational level, et al), the most possible channel of infection, the time of affirmation and so on. To extract HIV-1 RNA from their 10 ml of peripheral blood and amplified by RT-PCR. Positive PCR products were sequenced by Beijing Bomaide company after being purified. All of the sequenced were edit assembled into contiguous sequences. HIV genotype was determined by sumitting all of three genes to the NCBI viral genotyping tool(Bioedit, Simplot). All assembled sequence were alined with reference sequences using MEGA 5.05 for subtypes, phylogenetic analysis.Results 1. A total of 305 HIV infected patients were enrolled into the study.⑴The results show that there are more men than women in the chosen patients, male patients accounted for66.56%( 203/305),and female accounted for 33.44%(102/305),male to female ratio was 1.99:1(203/102).⑵ Patients aged ≥20 and ≤49 years old accounted for 56.72%(173/305)in total, and patients aged 50 years old or older accounted for 44.62%.⑶Farmers accounted for 50.82%(155/305). Secondly the housework and unemployed person accounted for 13.77%(42/305),workers and migrant proportion is 12.13%(37/305), Students and retired people respectively accounted for 2.3%(7/305) and 5.57%(17/305);⑷Patients with education level of junior school or below accounted for 76.39%(233/305), Students and retired people respectively accounted for 2.3%(7/305) and 5.57%(17/305);⑸The patients′s proportion who were transmitted through heterosexual contact is 95.74%. Same sex sexual transmission and the spread through injecting drug users accounted for a smaller proportion;⑹Married person accounted for 67.87%(207/305); the unmarried, divorced or widowed accounted for 16.72%(51/305) and 14.75%(45/305).2. The general situation of amplification and sequencingA total of 305 HIV infected patients `s peripheral blood, amplify gag region 175 cases successfully; Obtained gene sequence 155 cases successfully.3. The judgment and distribution of HIV-1 subtypes⑴155 specimens are gained in 305 specimens(amplification rate of 50.82%), then sequence analysis showed that there were 4 HIV-1 subtypes, CRF01-AE(124 samples),B(5 samples),CRF-07BC(12sample), CRF-08BC(14sample).⑵The areas were dominated by CRF01_AE, 5 cases of B subtypes distributed in the Guilin, CRF07-BC and CRF08-BC subtype distributed in every area.Conclusion 1.According to epidemiological data results, we can draw the following conclusions: to pay more attention to women with sexually transmitted diseases、the AIDS antibody screening of antemarital examination and pregnancy testing, to care the phenomenon of the elderly, students infected with AIDS through sexual contact, to intensify the supervision of the married patients,to take preventive measures to the low cultural level people.2.The gag sequence analysis showed that there were 4 HIV-1 subtypes, CRF01-AE(124 samples),B(5 samples),CRF-07BC(12sample), CRF-08BC(14sample), indicating the diversity of HIV-1 strains subtypes.3.The main route of transmission is the heterosexually transmission, the CRF01_AE is the major subtype in Guangxi. With the change of the main transmission and subtype, the more education and behavioral interventions should be done to prevent the spread of HIV/ AIDS.4. The study confirms that there are subtype diversity of HIV-1 strains and various routes of infection at molecular level in Guangxi area.
