Objective:By observing acute exacerbation of chronic obstructive pulmonary d-isease (AECOPD) with pulmonary hypertension (PH) in patients with coagulation f-ibrinolysis indexes, it aims to explore the changes of coagulation function combin-ed with clinical significance and correlation of diagnosis and treatment of pulmona-ry hypertension in AECOPD. Methods:248 patients with AECOPD were measured systolic pulmonary artery pressure (SPAP) by echocardiography. They were divided into 4 groups according to SPAP:normal PH group (≤30mmHg) 80 cases, mild PH group (31~50mmHg) 61 cases, moderate PH group (51~70mmHg) 57 cases a-nd severe PH group (≥71mmHg) 50 cases, coagulation fibrinolysis indicators and arterial blood gas analysis were detected from all the selected patients. Results:D-Dimer (D-D), platelets (PLT) in severe PH group were significantly higher than t-he moderate、the mild and normal ones; D-Dimer, PLT in moderate PH group we-re significantly higher than the mild and normal ones; while D-Dimer, PLT relativ-ely in between mild PH group and the normal group were not statistically signific-ant. Fibrinogen (FIB), PaCO2 in severe PH group and moderate PH group were si-gnificantly higher than the mild and normal ones; while FIB, PaCO2 in between s-evere PH group and moderate PH group、between mild PH group and normal PH group were not statistically significant. PaO2 in severe PH group and moderate PH group were significantly lower than the mild and normal ones; while PaO2 in bet-ween severe PH group and moderate PH group、between mild PH group and nor-mal PH group were not statistically significant. We found that D-Dimer, FIB, PLT levels have a significant positive linear relationship between PaCO2; While D-Di-mer, FIB, PLT levels were negatively correlated with PaO2; activated partial throm-boplastin time (APTT), prothrombin time (PT), thrombin time (TT) and PaCO2, Pa-O2 have no significant correlation. We also found that D-Dimer, PaCO2 levels have a significant positive linear relationship between PASP; While PaO2 was negatively correlated with SPAP. Conclusions:Monitoring patients with AECOPD, especially AECOPD with pulmonary hypertension in patients have an important valve in the change of FIB, D-Dimer, PLT, PaCO2, PaO2.
