Objective:To compare the efficacy and safety of ureteroscopy with Holmium:YAG Laser lithotripsy and extracorporeal shock wave lithotripsy in treatment of upper ureteral stones lcm or greater.Methods:We collected the clinical data of 133 cases who diagnosised single upper ureteral stones between March 2013 to December 2014,69 cases underwent extracorporeal shock wave lithotripsy(ESWL), and 64 cases underwent ureteroscopy with holmium:YAG laser lithotripsy(HL). The operation time,postoperative hematuria time,the successful rate of stone fragmentation,postoperative pain time,cost and hospital stay,clearance rate of stone after one month,all complications were ompared.Results:There is no significant statistical difference between the two groups in gender, age, medical history and the size of the stones. The average operation time and the hospital stay of HL were shorter than that of ESWL (40.31±5.33minVS49.06±15.47min, 6.19±1.68daysVS9.26±3.27days, P<0.05).Costs and successful rate of stone fragmentation and clearance rate of stone after one month were higher than ESWL(12791.3±898.1yuanVS6401.4±1596.4yuan,95.3%(61/64)VS72.5%(50/69),P< 0.05). But the complications were higher than ESWL.Conclusion:Both ureteroscopy with Holmmm:YAG Laser lithotripsy (HL) and extracorporeal shock wave lithotripsy (ESWL) are feasible and safe for upper ureteral stones 1cm or greater.
