Detection Of Lymph Node Metastasis In Cervical Lymph Node Metastasis Model Of Pyriform Sinus Carcinoma Using USPIO-enhanced MR Indirect Lymphography

Objective:Evaluation of the lymph node situation in patients with cervical carcinoma is essential for effective therapy. The aim of this study was to access the feasibility of uspio enhanced MRI for the detection of cervical lymph node metastasis in cervical lymph node metastasis model of pyriform sinus carcinoma and to investigate the cervical lymph node metastasis enhancement imaging feature。Methods:Twenty rabbits were randomly divided into tumor and inflammatory groups with each group containing 10rabbits. In the inflammatory group,0.5 ml egg yolk emulsion was injected into the mandibular muscle to induce an inflammatory reaction in cervical lymph nodes. In the metastasis group, VX2 tumor tissue suspension was transplanted into the pyriform sinus submucosa of 15 New Zealand rabbits under direct laryngoscope. One month after tumor and egg yolk injection using 1.5T MR scanner, MRI were performed before and after USPIO injection to observe the enhancement features. Size, enhancement pattern were evaluated on MRI images. Histopathological analysis was performed on all rabbits. USPIO enhanced MR imaging and histopathologic nodal stages were compared.Result:In the tumor group, the sensitivity and specificity of Plain MRI were 57.6% and 60.7%, respectively. The corresponding values of USPIO-enhanced MRI were 96.1% and 85.7%;The sensitivity and specificity of Plain MRI differ significantly from those of USPIO-enhanced MRI. In the hyperplasia group, the specificity of USPIO-enhanced MRI was 85.7%Conclusion:The features and extent of metastasis in the lymph nodes corresponded to those observed on USPIO-enhanced MR images. USPIO-enhanced MRI is useful for the detection and estimation of lymph node metastasis in cervical carcinoma.
