Objective To observe the effect of central lymph node dissection in cN0papillary thyroid carcinoma. To investigate the value of central lymph nodedissection in patients with cN0papillary thyroid carcinoma, and to conclude theintra and post operative treatment experience.Methods The clinical data of187patients with cN0papillary thyroidcarcinoma who had been performed thyroidectomy and central lymph nodedissection by the same operation group during their hospitalization from June,2012to December,2013in unit1general surgery of Chinese PLA general hospitalwere reviewed retrospectively. The postoperative situation was followed for aperiod of time.Result The central neck lymph node metastases rate was35.8%(67/187) inthis187cases study, which in the patients with aged less than45years, with thetumor diameter more than1cm, capsular or extracapsular invasion were45.05%,53.95%,94.74%,67.57%, significantly higher than those in patients with aged45years and above, with the tumor diameter1cm and below, and without capsular andextracapsular invasion. After the operation, there were32(17.1%) patients withtemporary hypothyroidism as well as4(2.1%) patients with temporary recurrentlaryngeal nerve injury, however, without any permanent recurrent laryngeal nerveinjury and permanent hypoparathyroidism cases. During the4-23monthsfollow-up, with the average13.4months,1cases of lateral neck lymph nodemetastasis was observed. He was recovered well after the reoperation.Conclusion The central neck lymph node metastases rate was associated with the age of patients, tumor diameter and T staging of the cancer rather than thegender of patients, mulifocality and the location of cancer. As an essential,effective and safe operation to the patients with cN0papillary thyroid carcinoma,central lymph node dissection can provide valuable staging information to thecancer and should be performed by experienced hands.
