Objective: To investigate the incidence of stroke associated pneumonia in acuteischemic stroke patients and identify its risk factors, and evaluate the diagnostic value ofA~2DS~2score applied in predicting stroke associated pneumonia in acute ischemic strokepatients. Methods: A retrospective analysis was performed for636acute ischemic strokepatients in three hospitals in Xinjiang from August2013to June2014. The incidence ofstroke associated pneumonia was calculated, the risk factors were identified by themethods of univariate and multivariate analysis. The diagnostic value of A~2DS~2score wasevaluated in predicting stroke associated pneumonia in acute ischemic stroke patients,according to the evaluation method of diagnostic test and the receiver-operatorcharacteristic curve. Results:①The incidence of stroke associated pneumonia was14.5%in acute ischemic stroke patients in three hospitals in Xinjiang.②According to theresults of univariate and multivariate analysis, male, age (≥60years), atrial fibrillation,dysphagia, consciousness disorders, hyperhomocysteinaemia, hypoproteinemia andNIHSS score≥15were associated with stroke associated pneumonia in acute ischemicstroke patients (P<0.05).③The optimal cutoff values of A~2DS~2score applied inpredicting stroke associated pneumonia in acute ischemic stroke patients, acute ischemicstroke (anterior circulation infarction) patients and acute ischemic stroke (posteriorcirculation infarction) patients were all7, and the C statistics of A~2DS~2score were0.938(95%CI:0.910-0.966),0.937(95%CI:0.904-0.970) and0.925(95%CI:0.873-0.976),respectively. Conclusions: Male, advanced age, atrial fibrillation, dysphagia,consciousness disorders, hyperhomocysteinaemia, hypoproteinemia and NIHSS score≥15were independent risk factors for stroke associated pneumonia in acute ischemic stroke patients; the values of A2DS2score were high in predicting stroke associated pneumonia inacute ischemic stroke patients, acute ischemic stroke (anterior circulation infarction)patients and acute ischemic stroke (posterior circulation infarction) patients.
