BackgroudAs the advantage of real-time image, cheap, easy to use, can locate a position exactly, echocardiography is the most convenient and common used way to diagnose hypertrophic cardiomyopathy (HCM). HCM is a relatively frequent heart disease in clinical work and there is an absolutely familial aggregation. According to a large amount of material of recent years, the incidence of HCM is180/100,000in china, which almost equal to200/100,000in the world. Its pathological features are cardiomyocyte hypertrophy, uniquely shape and disorganized arrangement, and they are particularly apparent in the interventricular septum. HCM is generally recognized as an euchromosome dominant genetic disease. However, we can find some patients whose parents has no this kind of disease, and this is hard to explain. Some think these can be the promoting factors:impaired metabolism of circulating catecholamines, intracellular calcium dysregulation, hypertension vigorous aerobic activity and so on. Exploring the diagnostic characteristics of echocardiography has important clinical significance. As the advance of ultrasound and research technology, research on all respects of HCM has been made, including the ways of study, and all the respects of HCM, and goes deep. Nowadays, speckle tracing image (STI) is a technology which is recognized as a good and exact way to do research and is being hot used.2-dimensional speckle tracing image (2D STI) has been used by some researchers and has got some conclusions. Nevertheless, there are some limitations of it:it’s not the real time3dimensional, and plenty of steps are needed, and so on. With the development of science and technology,3dimensional speckle tracing image (3D STI) emerges and over comes the limitations of2D STI. So this study is made to explore the3D STI features of HCM.ObjectiveThe aim of the study is to assess the left ventricular(LV) global and regional systolic function in hypertrophic cardiomyopathy (HCM) patients by exploring the characteristics of strain with the real time3-Dimensional speckle tracing imaging(RT3D-STI).MethodsGlobal and partial longitudinal, circumferential, radial and area strain of20HCM patients and22healthy people were measured by3D-STI and compared.ResultsIn the HCM patients, the global and partial strain of LV were lower than those in the healthy people. Meanwhile, global strain significantly decrease in the longitudinal strain (P<0.01), radial strain (P<0.05), area strain (P<0.01).Partial strain significantly decreases in the septum, anterior, inferior at both mid-cavity level and apical region (P<0.01or P<0.05)ConclusionsThe global and partial strain of LV were lower than those in the healthy people. The real time3-Dimensional speckle tracing imaging offers a novel way to assess the systolic function of HCM.
