The Clinical Research Of Arthroscopic Debridement, Training Of The Quadriceps Muscle, Traditional Chinese Medicine Fumigation In The Treatment Of Knee OA

Objective:A comparative study of arthroscopic debridement (AD) and arthroscopic debridement with long-term training of the quadriceps muscle (AD-LTQ) and arthroscopic debridement with traditional Chinese medicine fumigation (AD-TCM) in the recent clinical curative effect of treatment of the osteoarthritis of knee (knee OA), and providing reference for clinical treatment of knee OA.Methods:90patients (90knees) from March2012to June2013in the department of the Teaching hospital of Chengdu university of T.C.M, treated by NSAIDs and other ways of non-surgical treatment with poor clinical curative effect, were all in accordance with arthroscopic debridement indications and suggested to arthroscopic debridement. According to the different methods of postoperative treatment of knee OA,90cases were divided into three groups, group A patients treatment by with AD; Group B patients treatment by with AD-LTQ; Group C patients treatment by with AD-TCM.30cases were collected in every group. Group A, male12and female18, aged41to74years, with an average of54.40years, courses ranged from3to9years, with an average of6.01years. Group B, male10, female20, aged39to72years, with an average of52.63years, course ranged from3to9years, with an average of6.00years. Group C, male17and female13, aged40to73years, with an average of56.73years, courses ranged from3to9years, with an average of6.30years. The age, gender, duration, preoperative X-ray, Kellgren-Lawrence classification, preoperative Lysholm scale among the three groups had no statistical significance (P>0.05).During treatment for5weeks,6months,1year, patients were reviewed in outpatient service or telephone follow-up regularly, and Lysholm criteria was used to evaluate the curative effect of knee joint function. SPSS19.0statistical software was used to analysis the data of Statistical results.Results:The function of knee joint in5weeks after AD-TCM was much better than AD-LTQ and AD. Quadriceps training didn’t have a good effect on The function of knee joint at5weeks after AD. However, quadriceps training on knee joint function was appeared after six months, which was equal to the AD-TCM. After six months, at the same time, the efficacy of traditional Chinese medicine fumigation on knee joint function had begun to decrease. Insist for a long time to quadriceps functional training for knee joint function was improved obviously and significantly higher than the other two groups in1year after AD, and1year later, the knee joint function of group with AD-TCM was worse and worse and was equal to AD as the time went on.Conclusion:In the three groups of experiments, three methods were effective methods for the treatment of knee OA. The efficacy of AD on knee joint function would decrease as the time went on. The function of knee joint in6months after AD-TCM was much better than AD-LTQ, but it would decrease after6months as the time went on. However, the Chinese medicine fumigation didn’t have a positive effect on knee joint function after1year. With the AD-LTQ, the knee joint function would be better as the time went on.
