Objective: To discuss the expression of EMMPRIN and HER-2in the serum of patientswith lung cancer and explore its effect on clinical diagnosis and therapy in patients withlung cancer.Methods: Under the premise of informed consent, the serum were collected in60patients of lung cancer with different pathological types before chemotherapy and20normal healthy adults in our hospital medical center for health examination, amongwhich60patients with lung cancer were given2cycles of chemotherapy treatmentaccording to the disease in the Department of Respiratory Medicine of Yijishan Hospitalaffiliated to Wannan Medical College from February2012to January2014. The serumconcentrations of EMMPRIN and HER-2were measured by using an enzyme-linkedimmunosorbent assay. Analysis the relationship between the serum concentration ofEMMPRIN and HER-2changes in patients with lung cancer、normal healthy adults.Analysis of the changes of the serum concentration of EMMPRIN and HER-2atdifferent pathological types of lung cancer.Analysis of the changes of the serumconcentration of EMMPRIN and HER-2before and after chemotherapy in patients withlung cancer.The date were analyzed using a statistical software(SPSS17.0). The databetween the two groups were compared with t test.Among groups were compared withsingle factor analysis of variance.Multiple groups two two compared with the studentNewman Keuls Test(SNK) test.Paired data were compared with Paired t test.Thecorrelations between variables were analyzed using the linear correlation test. ROC curve was used to analysis and evaluate the diagnostic value of EMMPRIN and HER-2.Results:1. The serum concentration of EMMPRIN and HER-2in lung cancer patientsgroup were (13.65±2.90ng/ml,4.50±0.93pg/ml),the serum concentration of EMMPRINand HER-2in normal healthy adults group were (8.26±2.91ng/ml,2.18±1.39pg/ml).The serum concentrations of EMMPRIN and HER-2in lung cancer patients group werehigher than those in normal healthy adults group, the difference was statisticallysignificance.2. The serum concentrations of EMMPRIN in non small cell lung cancer patientsgroup was14.66±2.70ng/ml, The serum concentrations of EMMPRIN in small celllung cancer patients group was11.63±2.17ng/ml,The serum concentrations of EMMPRINin non small cell lung cancer patients group was higher than it in small cell lung cancerpatients group. The difference was statistically significance; The serum concentrationsof HER-2in non small cell lung cancer patients group was4.78±0.93pg/ml,The serumconcentrations of HER-2in small cell lung cancer patients group was3.95±0.66pg/ml,The serum concentrations of HER-2in non small cell lung cancer patients group washigher than it in small cell lung cancer patients group. The difference was statisticallysignificance.3. The serum concentrations of EMMPRIN in lung adenocarcinoma patients groupwas15.08±2.71ng/ml,the serum concentrations of EMMPRIN in squamous cellcarcinoma of lung patients group was14.25±2.69ng/ml,the serum concentrations ofEMMPRIN in small cell lung cancer patients group was11.63±2.17ng/ml,the serumconcentrations of EMMPRIN in lung adenocarcinoma patients group compared with thesquamous cell carcinoma of lung patients group,the difference was no statisticallysignificance;the serum concentrations of EMMPRIN in lung adenocarcinoma patientsgroup was higher than it in small cell lung cancer patients group, the difference wasstatistically significance;the serum concentrations of EMMPRIN in squamous cellcarcinoma of lung patients group was higher than it in small cell lung cancer group, the difference was statistically significance;The serum concentrations of HER-2in lungadenocarcinoma patients group was5.07±0.93pg/ml,the serum concentrations ofHER-2in squamous cell carcinoma of lung patients group was4.49±0.85pg/ml,theserum concentrations of HER-2in small cell lung cancer patients group was3.95±0.66pg/ml,two two comparison, the difference was statistically significance;Thelevel of HER-2in the lung adenocarcinoma patients group was the highest, squamouscell carcinoma of lung patients group was the secondary, small cell lung cancer patientsgroup was the lowest.4. The serum concentrations of EMMPRIN in lung adenocarcinoma patients groupbefore chemotherapy and after chemotherapy were (15.08±2.71ng/ml,11.83±2.34ng/ml).The serum concentrations of EMMPRIN in lung adenocarcinoma patients group afterchemotherapy was lower than it before chemotherapy.The difference was statisticallysignificance;The serum concentrations of HER-2in lung adenocarcinoma patients groupbefore chemotherapy and after chemotherapy were (5.07±0.93pg/ml,3.99±1.31pg/ml).Theserum concentrations of HER-2in lung adenocarcinoma patients group afterchemotherapy was lower than it before chemotherapy.The difference was statisticallysignificance.5. The serum concentrations of EMMPRIN in squamous cell carcinoma of lungpatients group before chemotherapy and after chemotherapy were (14.25±2.69ng/ml,12.40±1.98ng/ml).The serum concentrations of EMMPRIN in squamous cell carcinoma oflung patients group after chemotherapy was lower than it before chemotherapy.Thedifference was statistically significance;The serum concentrations of HER-2insquamous cell carcinoma of lung patients group before chemotherapy and afterchemotherapy were (4.49±0.85pg/ml,3.64±1.27pg/ml). The serum concentrations ofHER-2in squamous cell carcinoma of lung patients group after chemotherapy waslower than it before chemotherapy.The difference was statistically significance.6. The serum concentrations of EMMPRIN in small cell lung cancer patients group before chemotherapy and after chemotherapy were (11.63±2.17ng/ml,10.29±2.22ng/ml).The serum concentrations of EMMPRIN in small cell lung cancer patients group afterchemotherapy was lower than it before chemotherapy.The difference was statisticallysignificance;The serum concentrations of HER-2in small cell lung cancer patients groupbefore chemotherapy and after chemotherapy were (3.95±0.66pg/ml,3.25±0.77pg/ml).Theserum concentrations of HER-2in small cell lung cancer patients group afterchemotherapy was lower than it before chemotherapy.The difference was statisticallysignificance.7. The area under the curve of EMMPRIN obtained by ROC curves was0.906,andthe critical point of clinical diagnosis was10.21ng/ml, the diagnosis sensitivity was91.7%, and the specific degree was75.0%. The area under the curve of HER-2obtainedby ROC curves was0.908, and the critical point of clinical diagnosis was3.53pg/ml,the diagnosis sensitivity was88.3%, and the specific degree was85.0%. The area underthe curve of EMMPRIN and HER-2combined detection was0.955, the combineddetection of EMMPRIN and HER-2diagnosis sensitivity was95.0%, the combineddetection of EMMPRIN and HER-2specific degree was85.0%.8. There were positive correlation between EMMPRIN and HER-2in the serum oflung cancer group and normal healthy adults.Conclusion:1、The serum concentrations of EMMPRIN and HER-2in lung cancer patientswere significantly higher than those in normal healthy adults. The serum concentrationsdetection of EMMPRIN and HER-2may have important significance in the diagnosis oflung cancer.2、The serum concentration of EMMPRIN and HER-2have a certain degreedifference in lung cancer patients with different pathological types.The serumconcentration of EMMPRIN and HER-2may contribute to the judgment of types of lungcancer. 3、The serum concentrations of EMMPRIN and HER-2in each group of lungcancer decreased after chemotherapy,compared with the prior to chemotherapy, thedifference was statistically significance.The serum concentrations of EMMPRIN andHER-2on the effect of chemotherapy in lung cancer patients may have a certain guidingsignificance.4、The combined detection of EMMPRIN and HER-2can improve the diagnosisefficiency, reduce misdiagnosis.5、There were positive correlation between EMMPRIN and HER-2in the serum oflung cancer group and normal healthy adults.In brief, detection of the serum concentrations of EMMPRIN and HER-2may havean important significance in the diagnosis and the treatment of lung cancer.They had thepossibility to use in the clinical diagnosis and the treatment of lung cancer as a newtumor marker.
