The Study Of NO.12p Limph Node Dissection In The Hepatoduodenal For Advanced Distal Gastric Cancer With D2Lymphadenectomy

Obiective: Study of advanced distal gastric cancer underwent D2radical scavengingmethod and significance of No.12p groups of lymph node surgery.Methods: A retrospective analysis of total193in our hospital from clinical andpathological data during the period from2011September to2013September in distalgastric cancer, which was performed D2radical distal gastrectomy or tatal gastrectomyand No.12p group lymph node dissection.Results: Borrmann Ⅲ, Ⅳ No.12p group lymph node metastasis rate was significantlyhigher than that of Borrmann Ⅰ, Ⅱ (χ2=4.841, P=0.028). Angle of stomach,gastricantrum and multiple cancer NO.12p lymph node metastasis rate were0,2.2%and19.6%. Multiple cancer lymph node metastasis was significantly higher than that ingastric angle(χ2=9.344, P=0.002), gastric antrum district(χ2=10.530, P=0.001). N0~1lymph node metastasis was lower than N2~3No.12p group (χ2=4.433, P=0.035). Therewas no metastasis group12p lymph nodes in the patients who had tumor without serosalinvasion.Tumor diameter <3cm，3~5cm and>5cm No.12p lymph node metastasisrate was0%,4.0%,12.5%. More than5cm of tumor metastasis No.12p group morelikely(χ2=6.581, P=0.010). There is a correlation between the positive rate was4to5group of lymph node metastasis were associated with NO.12p groups of lymph nodes.And NO.5group lymph node positive patients are more likely to occur in NO.12p grouplymph node metastasis. There is no statistically different for the NO.12p group lymph node metastasis effects with these factors, which is gender, age, tumor differentiation,tumor thrombus and tumor vascular nerve invasion.Conclusion: Distal gastrectomy with D2lymphadenectomy for cleaning NO.12p lymphnode is reasonable and necessary. Pathological factors and operation mode are crucial tothe NO.12p lymph node dissection.
