ObjectiveTo investigate the clinical value of free fatty acid(FFA) in patients with type2diabeticnephropathy (T2DN) and researched the relationship between the pathogenesis of T2DNand serum FFA further.MethodsStudy participants are individuals who involved in type2diabetes mellitus(T2DM),according to1999WHO diagnosis standard,and with reference to “type2Diabetes Guide2010”.They were all came from the ward and outpatient in Jinan CentralHospital from August2012to May2013. Total276cases.They were divided into T2DMgroup(76cases in male and72cases in female) and T2DN group(65cases in male and63cases in female) in terms of past medical history,the level of blood creatinine, blood ureanitrogen, urine microalbumin or clinical albuminuria.138healthy subjects as controlgroup(N group,72cases in male and66cases in female),.Subjects are excluded in thefollowing situations,such as severe function damage of liver or heart, acute and chronicrenal damage with other causes, thyroid disease, malignant tumor, operation or trauma,severe infection, anemia etc.Participants in each group were examined height(H),weight(W), blood pressure(BP),Then calculated the body mass index(BMI),andfasting blood-glucose(FPG),hemoglobin A1C(HbA1c),serum triglyceride (TG),totalcholesterol(TCH), high density lipoprotein cholesterol (HDL-C), low density lipoproteincholesterol (LDL-C), urine microalbumin (UMA), urine protein (UPro), uric acid(UA),urineβ2microglobulin(U2M), urea nitrogen(BUN), serum creatinine(Cr) were measuredrespectively. The data extracted from Hospital Information System(HIS) was exported toMicrosoft Excel2003database.The statistical data was treated by analysis of One-wayANOVA, simple linear correlation and multiple linear regression analysis. The resultswere analyzed by SPSS18.0software.Results The difference between three groups in age, body mass index, blood pressure had nostatistically significant(P>0.05).They were matched.FPG、HbA1c in T2DN group werehigher than that in N group and T2DM group (The difference with N group had statisticallysignificant). It suggests that DN is related to poor glycemic control. TG、TCH、LDL-C inT2DM group and T2DN group were higher than that in N group(P<0.05);HDL-C in T2DNgroup was lower than that in N group. Compared with N group, HDL-C decreased slightlyin T2DM, but the difference was not statistically significant. This study confirmed thatTG、TCH、LDL-C were increased in T2DM, conversely, HDL-C was decreased. Theresults of this study showed that the difference in FPG, HbA1c, TG, TCH, HDL-C andLDL-C between T2DN group and T2DM group were not statistically significant(P>0.05).FFA in N group, T2DM group and T2DN group was increased gradually,and thedifference between three groups had statistically significant. In the correlation analysis ofFFA levels with other factors in diabetic, the simple linear analysis was used to test thecorrelation of BUN、Cr、UA、UMA、U2M,and rank correlation analysis was used to testthe correlation of Upro.The results showed that FFA had a positive correlation withBUN、UA、UMA、U2M、Upro(The correlation coefficients were0.153、0.102、0.166、0.440、0.121、0.253),which had a statistically significant(P<0.05).FFA as the dependentvariable,the multiple linear regression of BUN、Cr、UA、UMA、U2M,the results showedthat only FPG and UMA into the regression equation,and played a role in the equation,P<0.05.Standardized regression equation:=0.210+0.131X1+0..498X2.FFA in T2DN andT2DM groups were significantly higher than those in normal control group, and FFA inT2DN group was higher than those in T2DM group(P<0.05). In the correlation analysis ofFFA levels with these factors, The results showed a positive correlation with BUN,Cr,UMA,U2M,Upro, which had a statistically significant(P<0.05), The correlation ofUMA and FFA was the largest one among them(r=0.440).Conclusion(1)Compared with the T2DM and normal control group,FFA had a significantlyincrease in T2DN group. The serum FFA may be involved in the occurrence anddevelopment of T2DN,and it also can be a mutually reinforcing process.FFA can be usedas an independent risk factor for the T2DN.(2)TG、 TCH、 LDL-C were increased in T2DM, conversely, HDL-C wasdecreased,and this characteristic promoted the development of pathogenesis in T2DM.(3)The detection of FFA in clinical practice,high accuracy,easy to operate,and spend less,which has important clinical value.
