Correlation Analysis Between Hypercoagulable State And Coronary Heart Disease In Elderly People

Objective:By detecting the levels of Fibrinogen(Fg)、Antithrombin-III(AT-III)、GranuleMembrane Protein-140(GMP-140) and Lipoprotein (a)involved in the patients, it canassess the situations of the hypercoagulable state inside their bodies, investigate thecorrelation between hypercoagulable state and coronary heart disease, and finallyevaluate if these factors could monitor and predict the coronary heart disease.Methods:130elderly patients were collected and divided into four categories, which werestable angina pectoris (SAP) group with30cases, unstable angina pectoris (UAP)group with41cases, acute myocardial infarction (AMI) group with29cases, and acontrol group with30patients, according to the clinical diagnostic criteria combinedwith some selective coronary angiography. Then, according to selective coronaryangiography in some patients of UAP group and AMI group,40patients weredivided into three-vessel group with18cases, double-vessel disease group with12cases, single-vessel group with10cases. detecting the levels of Fg、AT-III、GMP-140、Lpa of patients in each group, and employing the SPSS19.0to analyze theexperimental data.Results:1) By comparing the age, gender, hypertension, smoking, diabetes,hyperlipidemia and other common factors, it is illustrated that there was notstatistically significant difference between control group, SAP group, UAP group andAMI group according to the P value (P>0.05).2) By comparing the general parameters of Thrombelastography (TEG)inthese four groups, the R and K values in AMI and UAP groups were shorter than thatin SAP and control groups, but Ma value and Angle increased significantly (P<0.05). Although the each value in SAP group was higher than that in the control group, therewas no significant difference (P>0.05).3) The hypercoagulable graphic in AMI group and UAP group was significantlyincreased with (P <0.01).4) The levels of Fg、GMP-140and Lpa increase in sequence of SAP group, UAPgroup and AMI group. The level of AT-III decreases in sequence of SAP group, UAPgroup and AMI group. There was significant difference between AMI group and otherthree groups in each factor. There was also significant difference between UAP groupand SAP and control groups in each factor. There was difference between SAP groupand control group in aspect of GMP-140and Lpa.5) By comparing the age, gender, hypertension, smoking, diabetes,hyperlipidemia and other common factors, it is illustrated that there was notstatistically significant difference among the single-vessel disease group,double-vessel disease group and three-vessel disease group according to the P value(P>0.05).6) The levels of Fg, GMP-140, Lpa in three-vessel disease group were higherthan that in double-vessel disease and single vessel-disease group, but the level ofAT-III in three-vessel disease group were lower than that in double-vessel disease andsingle-vessel disease group. The differences among the three groups were statisticallysignificant (p <0.05).The levels of Fg、GMP-140and Lpa in the double-vessel diseasewere higher than that in the single-vessel disease with P<0.05. Although the level ofAT-III in the double-vessel disease group were lower than that in the single-vesseldisease group, there was no significant difference.7) Through the Gensini score correlation analysis by considering the levels of Fg,AT-Ⅲ, GMP-140and LPa, Fg is positively correlated with Gensini score (r=0.627, p=0.000); AT-III and Gensini scores are negatively correlated (r=-0.385, p=0.014);GMP-140and Gensini scores are positively correlated (r=0.472, p=0.002); Lp (a)and Gensini scores are positively correlated (r=0.544, p=0.000).8) By considering the pairwise correlation among the levels of Fg, AT-Ⅲ,GMP-140and LPa. It is found that Fg and AT-III are negative correlation (r=-0.642, p=0.000), Fg and GMP-140are positively correlated (r=0.674, p=0.000), Fg andLpa are positively correlated (r=0.715, p=0.000), AT-III and GMP-140arenegatively correlated (r=-0.832, p=0.000), AT-III and Lpa negative correlation (r=-0.849, p=0.000), GMP-140and Lpa are positively correlated (r=0.973, p=0.000).Conclusions:The hypercoagulable state plays an important role in the development of thecoronary heart disease. The increase of the levels of Fg、GMP-140、 and Lpa andthe decrease of the level of AT-III can affect the hypercoagulable state, therefore, todetect the levels of Fg、AT-III、GMP-140and Lp(a)can assess the hypercoagulablestate inside the body, and precdict the coronary heart disease.
