| Objective:To investigate blood levels of glycosylated hemoglobin (HbAlc), brain natriuretic peptide (BNP) and their correlation with carotid atherosclerosis in hypertension patients without diabetes.Methods:A total of225patients diagnosed hypertension without diabetes, who admitted in the Ningbo Long Sai Hospital or outpatient clinic from March2013to December2013, were enrolled in this research, of them,124were male,101were female. According to the result of carotid ultrasonography, they were divided into carotid atherosclerosis group (129patients) and carotid artery normal group (the control group,96patients). We recorded the patient’s gender, age, height, weight, smoking status, etc respectively. Blood pressures were detected twice in the early morning and average blood pressure was recorded, and then in the next morning, blood was extracted to detect the blood HbAlc, BNP and blood lipids, etc after at least8hours fasting. Meanwhile, adjusted OGTT test was taken including blood glucose of fasting and2hours after meal. The carotid artery ultrasound examination was done to determine the wall intima-media thickness (IMT) quantitatively and probe the formation of plaque. The patients with thicken IMT and carotid plaques were defined as carotid atherosclerosis group.Results:There was no significant difference in gender, age, height, body weight, MBI, smoking situation, diastolic blood pressure,2h post meal blood glucose and triglyceride between the two groups (P>0.05). Systolic blood pressure, blood HbAlc, BNP, fasting plasma glucose, total cholesterol and low density lipoprotein in carotid atherosclerosis group were higher than those in the control group (P<0.05). Multivariate logistic regression analysis indicated that carotid atherosclerosis was positively related with HbAlc (OR=2.23, P<0.05), and BNP (OR=1.05, P<0.01). Application of ROC analysis glycosylated hemoglobin, BNP prediction effect of carotid atherosclerosis, the area under the ROC curve of glycosylated hemoglobin is0.64and prediction of carotid atherosclerosis tangent point is5.5%(Youden index=0.214), the area under the ROC curve of BNP is0.80and prediction of carotid atherosclerosis tangent point is101.1ng/L (Youden index=0.523). Conclusion:Blood HbAlc and BNP levels is related to the atherosclerosis to some extent in patients with hypertension without diabetes and may be the risk factors of carotid atherosclerosis. |
