CRP And CEA In The Prognosis Correlation Research Of Non-small Cell Lung Cancer

Objective:L ung cancer is the most common malignancy worldwide, it is one of the most common diseases of respiratory medicine, and rather than small-cell lung cancer(NSCLC) accounts for 70-75% of all lung cancer incidence, C-reactive protein(CRP) is a kinds of acute phase protein(APP), CEA is a tumor marker, there are a lot of people will be CRP, CEA elevated in lung cancer patients, this study was designed to detect the serum CRP and CEA level of non-small cell lung cancer patients before treatment,and to explore its impact to the patients’ s survival and prognosis.Methods: Collected 113 patients of untreated non-small cell lung cancer from January 2013 to December 2014,all of them are diagnosed by pathologically,and the treatments to them are used by NCCN guidelines recommend first-line chemotherapy and according to the patient’s own conditions by palliative chemotherapy, follow-up and make a retrospective analysis of the factors that may affect the survival and prognosis.Results: The initial CRP concentration was within the elevated( ≥ 10mg/L) in 68 patients and normal(<10mg/L) in 45 patients. The initial CEA concentration was within the elevated(≥5ng/L) in 59 patients and normal(<5ng/L) in 54 patients.The pretreatment serum CRP level was correlated with the patient’s performance status(P = 0.014), maximum tumor diameter(P = 0.049),stage of disease(P = 0.002), pretreatment hemoglobin level(P = 0.037), and pretreatment serum albumin level(P = 0.005).Compared serum CRP≥10mg/L in patients to serum CRP <10mg/L in patients before treatment, the difference in median survival and median progression-free survival all have statistically significant.The pretreatment serum CEA level was correlated with the patient’s performance statu(P = 0.009), tumor stage(P = 0.024), and pretreatment serum albumin(P = 0.011). Compared serum CEA≥5ng/L in patients to serum CEA <5ng/L in patients before treatment, the difference in median survival and median progression-free survival also have statistically significant.The patients’ s survival and prognosis of patients was correlated with the pretreatment serum CRP level, CEA level, PS score, smoking status, the maximum tumor diameter, tumor stage, pretreatment hemoglobin, serum albumin therapy,and chemotherapy cycle.The pretreatment level of blood CRP value<10 mg/L and the CEA value<5ng/L compared with patients have elevated CRP or CEA patients, median survival and median progression-free survival was significantly longer, the differences all have statistically significant.So when the detection of CRP and CEA was combined,the level of the pretreatment serum CRP and CEA average in the normal range is an important index of good survival and prognosis to assess the non-small cell lung cancer patients.Conclusions: The level of the pre-treatment’s serum CEA and CRP may has a significant correlation to the survival and prognosis in patients with non-small cell lung cancer, the higher the level of CRP and CEA in the patient, the survival and prognosis is worse, the joint detection of CRP and CEA could improve the value of the assesement of survival and prognosis in non-small cell lung cancer, and when the lever of them were detected in the normal range, the median survival and the median progression-free survival will be significantly prolonged,that survival and prognosis will be better. So they may possibly be used as markers to assessment the patients’ s survival and prognostic.But it still need the further study of the larger sample.
