The Clinic Research Of Treating Hp-related Duodenal Ulcer By Kangfuxin Fliud Combined With Triple Therapy

Objective The thesis mainly research on the therapy of Kangfuxin liquid comblined with triple therapy to treat Hp-related duodenal ulcerera by studying the efficacy of clinical symptoms, ulcer healing and Helicobacter pylori eradication. Meanwhile, it makes appropriate assessments to the effectiveness, tolerability and safety of the treatment and provides evidence-based medicine basis for the treament of duodenal ulcerera by Integrative medicine therapy.Methods Randomly selected 88 patients who suffered from Hp-positive active duodenal ulcer and blood-stasis obstruction and got treatment in department of gastroenterology in Wuhan General Hospital of Guangzhou Military Region. Group A: 30 patients were treated with amoxicillin(1g,po,bid)、Clarithromycin SustainedRelease Tablets(500mg,po,bid) and esomeprazole magnesium enteric-coated tablets(20mg,po,bid) for one week, one week later continue to treat with someprazole magnesium enteric-coated tablets(20mg,po,qd) for 3 weeks. Group B: 29 patients were treated with Rebamipide(100mg,po,tid) for four weeks combined with the medicines in Group A. Group C: 29 patients were treated with Kangfuxin liquid(10ml,po,tid) for four weeks combined with medicines in Group A. Follow-up visits once a week during treatment to understand the patient’s compliance, clinical symptoms and adverse changes. 4 weeks after the treatment, detect Hp and observe the Hp clearance. Take serum samples to do blood routine examination, liver function, kidney function and other laboratory tests before and after the treatment. Follow-up 6 month after the treatment to observe the relapses of patients.Results ①Comprehensive clinical efficacy After 4 weeks of treatment, the cure rate in Group C increased compared with group A and the difference was statistically significant(p<0.05); Both the total efficiency and cure rate of Group C increased compared with Group B, but the difference was not statistically significant(p>0.05).②The symptom scores comparison of three groups before and after treatment: the clinical symptom scores of all the three groups decreased compared with the ones before treatment and the differen-ces were statistically significant(p<0.05).The symptom scores comparison of three groups after treatment: the symptom scores of Group C,such as abdominal pain,bloating,cold extremities decreased compared with the ones of Group A and the differences were statistically significant(p<0.05); the symptom scores of Group C, such as abdominal pain, cold extremities decreased compared with the ones of Group A and the differences were statistically signi-ficant(p<0.05).The efficacy of clinical symptoms comparison of three groups: after two weeks of treatment, the cure rate and total efficacy of group C were higher than the ones of Group A and the difference was statistically significant(p<0.05); the cure rate of group C were higher than the ones of Group B and the difference was statistically significant(p<0.05).After four weeks of treatment, the cure rate of both Group B and Group C are higher than the one of Group A and the differences were statistically significant(p<0.05).③After four weeks of treatment, the cure rate in group C increased compared with Group A and the difference was significant(p<0.05).④The Hp eradication rate in Group C increased compared with both Group A and Group B,but the difference was not statistic-ally significant(p>0.05).⑤1 patient in Group B and 1 patient in Group C stop taking their medication due to unable to tolerate the antibiotic drugs.⑥Obeserve after 6 months of treatment, there are no patients in Group A and Group C relapse. One patient in Group B relapse and the difference was not statistically significant.Conclusion The therapy of Kangfuxin liquid comblined with triple therapy to treat Hp-related duodenal ulcerera with blood-stasis obstruction has good efficacy and is safe. Though total efficiency and Hp eradication rate of the therapy have little difference from the ones of triple therapy, the therapy can improve the cure rate and rapidly relieve clinical symptoms of the patients and significantly improved the clinic symptoms, such as abdominal pain, bloating, cold limbs. Compared with Rebamipide triple therapy, it can improve the cure rate in two weeks of treatment, especially good at releasing clinical symptoms of bloating and cold limbs.
