| Objective: To observe the clinical effect of bone cement PKP( percutaneous kyphoplasty) treatment of OVCF(osteoporotic vertebral compression fractures), in order to provide some references for choose a effective and safe the amount of cement.Methods: All cases of OVCF from January 2013 to December 2014 treated spine surgery from our hospital with a total of 61 patients(69 vertebrae). male and female, 13 cases and 48 cases respectively, patients’ age range is 50 ~ 83 years old, average age is(67.39±9.59)years old. All patients were randomly divided into two groups, group A was low dose group with a total of 27 patients(34 vertebrae), bone cement perfusion measure 6 ml or less, group B was high dose group with a total of 34 patients(35vertebrae), bone cement filling quantity > 6 ml. By comparison: different groups patients’ general information, anterior vertebral height, central height, VAS score,leakage rate was no significant difference, had a good comparability. Analysis of two groups the difference between the amount of cement, VAS scores, anterior vertebral height, central height, recurrent fractures,and bone cement leakage.Results:1.The visual analog scale(VAS)In group A and group B patients on the first day of the VAS score improved significantly, before surgery group A VAS score was(7.89±0.93) points, after surgery was(2.59±0.93) points, the T value is 24.19, with significant difference(P < 0.01);before surgery group B VAS score was(7.97±1.17) points, after surgery was(2.53±0.96)points, T value of 20.33, with significant difference(P < 0.01). Therefore, the results show that the difference between groups with no significant(F = 0.064, P > 0.05). Two groups of relieve pain symptoms difference was no statistically significant.2. anterior vertebral height, central height Group A: before surgery the anterior vertebral height, central height was(16.64±0.78)mm,(18.51±0.86) mm respectively, after surgery was(20.05±0.97) mm,(22.51±1.17)mm; Compared the before surgery and after surgery, a significant difference(P < 0.01).Group B: before surgery the anterior vertebral height, central height was(16.82±1.02)mm,(18.73±0.90) mm respectively, after surgery was(20.20±0.97) mm,(23.30±1.05)mm. Compared the before surgery and after surgery, a significant difference(P < 0.01).compared with preoperative have obvious recovery. Two groups of anterior vertebral height, central height difference was no statistically significant(P > 0.05).3. Bone cement leakage In group A was low dose group with a total of 27 patients(34 vertebrae), 3 cases(3vertebrae) occured leakage, the occurrence rate was 8.8%; In high dose group with a total of 34 patients(35 vertebrae), 5 cases(5 vertebrae) occured leakage, the rate was14.3%. Two groups of bone cement leakage difference was no statistically significant(P > 0.05).4.Recurrent fractures In group A was low dose group with a total of 27 patients(34 vertebrae), recurrent fractures occurred in two cases(2 vertebrae), the recurrent fractures rate was 5.9%; In high dose group with a total of 34 patients(35 vertebrae), 3 cases(3 vertebrae) occured recurrent fractures, the recurrent fractures rate was 8.6%. Two groups of recurrent fractures difference was no statistically significant(P > 0.05).conclusion:PKP surgery can significantly reduce the pain of patients, anterior vertebral height, central height, in the current treatment of OVCF has a very good prospect,specific safety and high effective. Two groups of relieve pain symptoms, anterior vertebral height, central height, bone cement leakage, recurrent fractures difference was no statistically significant. |
