Objective:To observe the various complications of fetal distress and their impacts on maternal and child,and to explore the related factors for fetal distress and the influence of fetal distress on the mode of delivery, maternal outcome of pregnancy.To provide clinical evidence for the further strengthening of perinatal management of fetal distress and improve neonatal outcome.Methods: Using large sample of retrospective analysis method,2091 cases of pregnant women deliveried in obstetrics of Dalian Medical University Second Affiliated Hospital from October 2009 to October 2014, in which we selected 100 cases of single fetal head position, primipara and were diagnosed with fetal distress as observation group;choose other no fetal distress, single fetal head position, primipara normal pregnant women who were randomly selected at the same time in hospital as control group.The two groups in the age of the pregnant women, childbirth weeks of gestation, a fetus’ s sex had no statistical difference. Collect the following data,including:1, the risk factors of fetal distress:abnormal umbilical cord, oligohydramnios,,premature rupture of membrane, abnormal labor, gestational hypertension, gestational diabetes, etc; 2, the impact on the mother: delivery methods(including cesarean delivery, forceps midwifery), postpartum hemorrhage, etc; 3, newborn situation:preterm birth, birth weight and neonatal asphyxia(including mild and severe asphyxia),etc. The above results for case-control analysis. 4, Individual research in observation group the incidence of fetal distress during the different labor time.With the t test, x 2test method to study fetal distress in the outcomes of pregnant women and the influence on the newborns.Results:1,Observation group 45 cases abnormal umbilical cord, oligohydramnios 37 cases, 76 cases of premature rupture of membranes and abnormal labor 15 cases, 4 cases of gestational hypertension disease, gestational diabetes in 5 cases; Control abnormal umbilical cord 27 cases, 12 cases of oligohydramnios, premature rupture of membranes in 11 cases, lasting anomalies in 5 cases, gestational hypertension disease, 2 cases, 2cases of gestational diabetes.After statistics test, observation group of abnormal umbilical cord, oligohydramnios, amniotic fluid contamination, the incidence of labor abnormalities were significantly higher than that of control group(P < 0.05),statistically significant difference; Observation group and control group in gestational hypertension incidence of disease, the incidence of gestational diabetes, there was no statistically significant difference(P > 0.05).2,Observation group of 52 cases of pregnant women cesarean delivery, forceps midwifery 13 cases,3 cases of postpartum hemorrhage; Control group of 25 cases of cesarean section, forceps midwifery in 5 cases, 2 cases of postpartum hemorrhage,statistical results show that the observation group of cesarean section rate and forceps midwifery rate is higher than the control group(P < 0.05), the observation group and control group in the incidence of postpartum hemorrhage no statistical difference(P >0.05).3,Observation group of premature 8 cases, 10 cases of low birth weight and neonatal asphyxia 8 cases; Control premature in 1 case, low birth weight in 2 cases, neonatal asphyxia in 1 case, and observation group of premature and low birth weight and neonatal asphyxia were higher than the control group(P > 0.05), the difference was statistically significant.4,Individual research in observation group found that the highest incidence of fetal distress in the second labor.Conclusion:Fetal distress seriously affect the perinatal health, and have an effect on the mode of delivery.Fetal distress can occur in the production process in any time, and in the second stage of labor has the highest incidence rate.There are many factors that can cause the high risk of fetal distress, clinical should attach importance to it, and comprehensive consideration, comprehensive evaluating fetal condition, timely,accurate judgment the occurrence of fetal distress and correct treatment is key to improve maternal adverse outcome.
