Expression And Clinic Significance Of HER-2 In Gastric Carcinoma

Background: Gastric cancer is one of the common malignant tumors in China,a serious impact on human health, gastric cancer involves activating mutations and deletions of tumor suppressor genes and oncogenes. In normal circumstances Human epidermal growth factor receptor 2(Her-2) inactive, when the Her-2 be exposed to some carcinogens, which is activated by the Changes in the structure or uncontrolled expression,so has active to transformed tumor,lead to malignant transformation of cells. The study found Her-2 protein also overexpression in gastric cancer,Can provide a theoretical basis for the targeted treatment of gastric cancer. Objective:To observe the expression of human epidermal growth factor receptor 2(Her-2) in gastric cancer tissue,and to explore the association of Her-2 in the pathogenesis of gastric cancer,provide a theoretical basis for the targeted treatment of gastric cancer. Methods:The expression of Her-2 protein was determine with immunhistochemistry(IHC) in 148 patient with gastric cancer.The relationship between Her-2 protein expression and sex,age,depth of tumor invasion, TNM stage,tumor size,tumor location and lymph node metastasis were analyzed. Results:(1) Expressive rate of Her-2 protein in 148 cases of gastric carcinoma tissue was 18.9%( 28/148), and in 20 cases of adjacent normal tissue of the stomach was not detected the expression of Her-2,the difference has statistical significance(P<0,05).(2)The expression of Her-2 in gastric cancer tissue:expressive rate of Her-2 with men was 16.5%,expressive rate of Her-2 with women was 24.4%(X2=1.287,P=0.257);expressive rate of Her-2 with less than 60 years of age patients was 15.9%,expressive rate of Her-2 with more than or equal to 60 years of age patients was 21.5%(X2 =0.747,P=0.387);expression rate of Her-2 with tumors less than 5cm was 23.4%,expression rate of Her-2 with trnnors larger than or equal to 5cm was 16.8%(X2=0.903,P=0.342);expression rate of Her-2 with lymph node metastases was 24.3%,expression rate of Her-2 without lymph node metastases was 6.7%(X2=6.569,P=0.01);expression rate of Her-2 with high differentiation was 17.4%, expression rate of Her-2 with middle differentiation was 17.9%, expression rate of Her-2 with middle-poorly differentiation was 17.2%, expression rate of Her-2 with poorly differentiation was 25%(X2=0.840,P=0.840);expression rate of Her-2 with TNM tumor stage I、II was 7.3%, expression rate of Her-2 with TNM tumor stage III、IV was 23.4%(X2=4.976,P=0.026);expression rate of Her-2 with T1+T2 depth of invasion was 10.6%,expression rate of Her-2 with T3+T4 depth of invasion was 25.6%(X2=5.366,P=0.021);expression rate of Her-2 in patients with gastric cardia department was17.4%, expression rate of Her-2 in patients with gastric body department was 17.6%,expression rate of Her-2 in patients with gastric sinus department was 19.8%(X2=0.115,P=0.944)。 Conclusion:(1)Her-2 Overexpression in gastric cancer,is not expression in the adjacent normal tissues.(2)There was excessive expression of Her-2 in gastric cancer tissue, excessive expression of Her-2 patients with cancer of the stomach had no correlation with the sex,age,, tumor size, tumor differentiation degree, tumor location; it was correlated with the presence of lymph node metastasis、tumor invasion、tumor TNM staging.
