| Objective:In order to find safer and more effective treatment of traditional Chinese medicine,clinical research are made through contrast observation of acute gouty arthritis patients which are divided into two goups.All the patients are given modified Simiao Decoction as the oral treatment,one group are given Electric acupuncture for treatment,the other used Voltaren ointment for cure.Then caculating clinical efficacy,conducting the detection of biochemical indices,detecting safety of the treatment and recurrence rate three months later of each group.Method:Among 60 cases of acute gouty arthritis patients,30 cases are randomly divided into treatment group(EA group),the other are looked as control group(Voltaren group), 30 cases in each group. All patients drink modified Simiao Decoction as basic treatment twice a day after breakfast and supper, every 200 ml, 7 days for a course of treatment, continuing two courses. EA group acupuncture Zusanli, Sanyinjiao, Hong Leong and the affected area with Ashi, then connecting to low frequency pulse of G6805 electroacupuncture apparatus by the frequency of 14~26Hz as stimulation parameters which lasting 30 min once a day. Voltaren group are given ointment outside the rub, of which the size is determined by sore dose, each with about 3-5cm or more three times a day,gently rubbing ointment so that it can penetrate into the skin and absorb quickly, every 7 days for a course,continuing two courses.Then observing clinical efficacy and safty,recording first analgesic effective time and maintain time,measuring changes in serum uric acid CRP, ESR values before and after treatment, and checking the recurrence rate after 3 months as all the treatment are finished.Conclusion:1. Clinical efficacy comparison: Compared two groups of patients’ symptoms and signs before the treatment,there had no significant difference(P> 0.05); compared each two groups’ symptoms, signs indicators after treatment,the result show statistically significance(P <0.05); after the treatment comparison between the two groups the indicators of EA had significant differences(P <0.05). 2.Comparison of analgesic last time: two groups’ analgesic onset time had no significant difference(P>0.05); compared with Voltaren drug group,analgesic time of EA can maintain longer,indicating a big significant difference(P<0.05). 3. Comparison of laboratory parameters: Before the treatment, the rate of uric acid,ESR and CRP of two groups of acute gouty arthritis patients were comparable,(P> 0.05);after treatment, each group showed big changes on erythrocyte sedimentation, C reactive protein and uric acid compared with before(P <0.05); between two groups,varies of these laboratory parameters presented uncomparable after treatment(P <0.05). 4.After the treatment the overall effect of the two groups comparison: After the treatment, in EA group, 12 cases recovered, 12 cases improved, 6 cases were ineffective, the total effective rate was 85%; in Voltaren group, nine cases cured, 12 cases improved, 9 cases were ineffective,the total effective rate was 70%. 5.3 months later two groups of patients were followed up, the recurrence rate comparison: In Voltaren group, five cases relapsed, Account for 16.6 percent of the total number; in EA group, nobody recurred, which made up the percentage of the total number of 0%. 6.Security Comparison: Compared liver and kidney function of the respective two groups before and after therapy, there was no statistical difference(P> 0.05); liver and kidney function prompted no much significance between two groups after treatment,(P> 0.05). Adverse reaction rates in Voltaren group were respectively 3.33% and 10%, otherwise in EA group there had not any adverse reaction occur.Result:1.EA combined with Modified Simiao decoction can significantly improve the heat Accumulation gout arthritis patient’s condition. 2.EA combined with Modified Simiao decoction group(treatment group) and Voltaren(control group) can improve heat Accumulation gout arthritis patient’s condition, lower blood uric acid, C-reactive protein and erythrocyte sedimentation rate values to extent, but the EA group is significantly better than the Voltaren group; and the EA group’s duration of analgesia for the first time after treatment can last longer, the recurrence rate is lower, and safer.3.The treatment of EA combined with Modified Simiao decoction on heat Accumulation gout arthritis patients have a good effect, EA can relatively reduce the application of NSAIDs and the incidence of adverse reactions in the clinical thrapy;it is worth promoting,because it have better long-term efficacy and it is safer for the patients,that can easily accepted by gout patients. |
