| Objectives With the constant progress of the Times, the development of science and technology, how to cure shin long bone defect caused by high-energy trauma becomes orthopedic clinical doctor’s problem. In the 1950 s, the former Soviet union scientist Ilizarov invented circular external fixator and through a lot of clinical practice and scientific research eventually formed the tension-stress rule, then originally invented bone removal technology in the 70 s. Through the clinical practice of research both at home and abroad, Ilizarov bone segments sliding technique can effectively cure long bone defect in tibia. But the factors that can be associated with the efficacy of treatment of long bone defects which are used Ilizarov’s bone segments sliding technique are less reported.This topic is about that the application of the bone segments sliding technique to treat Shin long bone defect that ranges with in 6~10 cm is retrospectively analyzed and summarized to explore.the clinical treatment effect of the long bone defect in tibia which is used Ilizarov’s segments sliding technique and related factors analysis to guide the clinical workMethods From January 2008 to June 2014, 63 cases of patients with long bone defect in tibia were retrospectively analyzed: Patients that be used the Ilizarov’s segments sliding technique on the treatment of the tibia bone defect in the upper in 28 cases make up group A, Patients that be used the Ilizarov’s segments sliding technique on the treatment of the tibia bone defect in the lower in 35 cases make up group B.By SPSS16.0statistical software, we analyze the relevant variables of the two groups including the age,sex, bone defect range, bone cutting plane and assess and statistically analyze calf function and the ankle-knee function of the A, B two groups removed external fixation after 1 month, 2 months, 3 months, 6 months.Result Average follow-up visit of 63 patients after operation is15~24 months. The tibia bone defect average length is 6 cm to 10 cm. Extended day is100~150 days. Tearing open external fixate time is 8~12 months, healing index 35 to 41 days/cm.all defects of bone healing, healing time is 7 months~23 months. Bone defects of all patients healing,patients after removal of ring type external fixator, ankle and knee function can obviously recovered.Conclusions 1 The effect in the treatment of long bone defects in tibia in which we use Ilizarov’s bone segments sliding technique after the operation is remarkable.2 Patients with large section of the bone defect in tibia be removed after the operation of Ilizarov’s bone segments sliding of external fixator, through the functional exercise can be restored to health and its bad leg has no obvious difference with its health crural nomal function.3Ilizarov’s technique of the segments sliding treatment of long bone defect in tibia bone can be found that as the growth of the bone defect length, Knee and ankle function after be removed of external fixator become worse and worse.4 Patients’ s ankle-knee function is worse after the treatment of long bone defect in tibia by Ilizarov’s bone segments sliding technique as the growth of the age,after be removed external fixator.5 Patients’ s knee-ankle function whose cutting plane of the shinbone bone damage which cured by Ilizarov’s bone segments sliding technique is under the tibia is worse than on the tibia knee-ankle function. |
