Objective:Improved surgery path of breast cancer, dynamic detecting cytokeratin 19 expression in peripheral blood of breast cancer patients under different surgical path.Find the best surgery path of breast cancer, providing a clinical scientific evidence for breast cancer micrometastasis.Methods:This study is prospective randomized controlled.Randomly selected pathologically confirmed of 80 patients with breast cancer(stage I-IIIa) whom were randomly divided into experimental group and control group of 40 patients,respectively.Experimental group improved surgical and control group underwent conventional surgery.Expression of cytokeratin 19 in all of the patients of peripheral blood were examined by flow cytometry one week before operation,during operation and one week after operation.EDTA was used as the anticoagulant and RBC lysis buffer was used to obtain leukocyte,then adding CK19-FITC.Obtained data were analyzed using t-test analysis(SPSS statistical software.Version 17.0 for windows).Results:The expression of CK19-positive cells in peripheral blood of patients between experimental group and the control group were statistically significant before and after operation and during operation,P<0.05.There were 15 lymph node-positive patients and 25 lymph node-negative patients in the control group,and the expression of CK19-positive cells in peripheral blood of patients between lymph node-positive patients and lymph node-negative patients were no statistically significant before and after operation and during operation,P>0.05.There were 13 lymph node-positive patients and 27 lymph node-negative patients in the experimental group,and the expression of CK19-positive cells in peripheral blood of patients between lymph node-positive patients and lymph node-negative patients were statistically significant before and after operation, P<0.05,but no statistically significant during operation,P>0.05.The expression of CK19-positive cells in peripheral blood of patients between15 lymph node-positive patients of the control group and 13 lymph node-positive patients of the experimental group were no statistically significant before and after operation and during operation,P>0.05.The expression of CK19-positive cells in peripheral blood of patients between 25 lymph node-negative patients of the control group and 27 lymph node-negative patients of the experimental group were statistically significant before and after operation and during operation,P<0.05.The two surgical methods were no significant difference in postoperative complications.Conclusions:The improvement surgical compared with the traditional surgical, the expression of CK19-positive cells in peripheral blood of patients were statistically significant in this study.The improved surgical procedures had a good effect in reducing breast caner micrometastases,whereas no significant difference in the two surgical procedures in handing and safety.Improved surgical procedures of Auchincloss can be improved in clinical to reduce the impact of surgical approach and operation for breast cancer micrometastases.
