Objective:To investigative the influence of neurological recovery with early versus delay decompression after acute spinal cord injury(SCI) in rats, and evaluate the effect of the timing of decompression after spinal cord injury on neurological recovery.The study’s purpose is to find what’s the time to decompress for guide clinical treatment. Methods:We used 68 healthy female SD rats, weighing 250-300 g, which were randomly divided into four groups: only laminectomy surgery(sham group A)、decompression surgery after six hours of SCI and oppression(group B)、decompression surgery after forty-eight hours of SCI and oppression(group C)、continuous compression(group D). Each above group was randomly divided into four group in accordance with the time after surgery(1d, 7d, 14 d and 28d), each rat would be evaluated by BBB score before and after surgery, and then remove the damaged segments of spinal cord to detect chemical activity changes in neurofilament protein(neurofilaments, NF), detect apoptosis by TUNEL, make the HE staining. Results:1、BBB score:The model of Sprague-Dawley rats with acute spinal cord injury and postoperative 1 day, BBB score of rats are 2, no significant difference between the groups(P>0.05);Postoperative 7、14、28d,BBB score of experience groups’ rats are significantly lower than group A,there are statistically significant differences(P<0.05);In experience groups,BBB score of group B is the highest, BBB score of group D is the lowest.2、Histological observation2.1 Specimens: The injured spinal cord edema and middle sag; sagittal diameter of the injury spinal cord was recovery after decompression.2.2 H-E Staining:Spinal cord of Group A was not injury;The degree of spinal cord injury was very sharp in group D, organizational structure displayed disorder, lesion cavity, surrounding format scar; the degree of the other three groups injury was ease relatively. the degree of damage in group B was easer than that in group C, spinal damage zone was no lesion cavity, no scar, no boundaries with the surrounding tissue, group C after spinal cord injury were few vacuoles cells, scar formation around.3、T U N E L Staining: Group A was almost no positive cells around, the other groups were seen lots of positive cells, uneven distribution.In group B, the positive cells were more in postoperative 7d than others, group C and D, the number of positive cells were gradually decrease with time miss, positive cells were still exist at postoperative 28 d.The number of positive cells were significantly reduced in group B than other two groups at the same time, and the number of positive cells in group B were lowest, Group D were most, there were statistically significant differences(P<0.05).4、Immunofluorescence staining: Group A was very few NF- 200 positive cells.The other groups, NF positive cells increase gradually by time miss, average absorbance increasely. Postoperative 1d,B、C、D has no significant differences. Postoperative 7、14、28d, Group B was highest of NF- 200 expression, group D positive cells number were least, there were statistically significant differences(P<0.05). Conclusions:1、Surgical decompress promote nerve function recovery in rats of acute SCI and compression.2、Nerve function recovery gets recovery more quickly from early surgical decompression than late surgical decompression.
