Effects Of Yougui Pill On Steroid Resistant Nephritic Syndrome Patients’ Immune Funtion And CORT、ACTH

Objective To observe the effects of Yougui Pill on steroid resistant nephritic syndrome patients’ immune function and CORT,ACTH, and to explore its role, which has strengthened the sensitivity of glucocorticiod and its mechanism of the role.Methods During the period from January 2014 to January 2015, a total of 40 cases of steroid resistant nephrotic syndrome patients were enrolled in the study. In the randomized controlled trial, they were randomly divided into two groups: treatment group(20cases) and control group(20cases). Control group was treated with western medicine, treatment group was treated with western medicine and Yougui Pill. The serum of immunoglobulin(Ig G,Ig A,Ig M) and complement(C3,C4) levels were detected by immunoturbidimetric assay.The levels of T lymphocyte subsets were analyzed by flow cytometry.The serum CORT,ACTH levels were detected by radioimmunoassay.Ig G,Ig A,Ig M,C3,C4,CD3+,CD4+,CD8+,CORT and ACTH content in blood of the two groups were observed before treatment and four weeks after treatment,while being recorded urine protein in 24 hours,ALB,Scr and clinical symptom score in TCM.Results 1. The western medicine clinical therapeutic effect of the treatment: there are 19 effective cases and 1 invalid case in treatment group, 14 effective cases and 6 invalid cases in control group, the total effective rate of control group was 70.00%, of treatment group was95.00%. There was significant statistical difference between the control group and the treatment group(P<0.05). 2. The Chinese tradtional medicine clinical therapeutic effect of the treatment: there are 20 cases and 0 case in treatment group, 16 cases and 4 cases in control group, the total effective rate of control group was 80.00%, of treatment group was 100.00%.There was significant statistical difference between the control group and the treatment group(P<0.05). Compared with control group,the condition of recovery in treatment group such as urine protein in 24 hours, ALB,Scr and clinical symptom score in TCM, the difference was a statistic difference(P<0.05). 3. the levels of serum immunoglobulin Ig G,Ig A,Ig M and complement C3,C4: In treatment group, the levels of Ig G,Ig A,C3 were higher and Ig M was lower than those in control group after treatment,while the difference of Ig G,Ig M in thesedata between two groups were distinctive(P<0.05). 4. Tlymphocyte subsets:the serum of CD3+%, CD4+%, CD4+/CD8+ levels were decreased and CD8+ level were increased in treatment group and control group before treatment, there is no markerly difference of these data in two groups(P>0.05). The levels of CD4+%, CD4+/CD8+ were higher and CD8+ was lower in treatment group than those in control group after treatment, there was significant statistical difference between the control group and the treatment group(P<0.05). 5. The serum of CORT and ACTH levels: CORT and ACTH levels were decreased in two groups before treatment, there is no markerly difference of these data in two groups(P>0.05). The levels of CORT and ACTH were higher in treatment group than those in control group after treatment, there was significant statistical difference between the control group and the treatment group(P<0.05).Conclusion Yougui Pill has markedly strengthened the sensitivity of glucorticoids to treat steroid resistant nephrotic syndrome.The mechanism of Yougui Pill, which has strengthened the sensitivity of glucocorticiod in treatment of SRNS, is possibly relevant to the function of NEI.
