Objective:Gamma Knife radiotherapy for preoperative and postoperative prevention control of metastatic encephaloma development in order to improve the effect of Gamma Knife radiation therapy and the management of postoperative patients.Methods:Retrospective analysis of the Gamma Knife Treatment Center of which the Second Hospital of Shanxi Medical University,from 2013 to 2014, application of Gamma Knife radiosurgery for brain metastases in 78 patients. Discussing PTBE and tumor size whether Gamma Knife radiosurgery have an effect on brain metastases and peritumoral brain edema after Gamma Knife radiosurgery.Results:1. There are differences among them(p<0.05) that postoperative different tumor size(large, medium and small) for postoperative local control rate(effective). This suggests that the larger tumor, which after Gamma Knife radiotherapy, the worse prognosis.The smaller the tumor size, on the contrary side,the better prognosis.2. There are differences among them(p<0.05) that preoperative different tumor size(large, medium and small) for postoperative local control rate(effective). This study showed that the smaller of preoperative tumor size, the situation of the postoperative tumour under control will be better than the largers.In other words, The larger ofpreoperative tumor size, the easier to stable after Gamma Knife radiosurgery. However,the smaller of preoperative tumor size, the easier to complete remission.3. Preoperative brain metastasis had different volume(large, medium and small)affect the prognosis of tumor weeks edema was statistically significant(p< 0.05). The results showed that: The smaller the size of brain metastases which before the gamma knife radiation therapy, the lighter degree of PTBE; on the contrary the greater the volume of brain metastasis, the higher the degree of PTBE.4. Before the gamma knife radiotherapy,different degrees of PTBE(none, mild,moderate, and severe) have an impact on brain metastases growth control(p<0.05),The results show that for the different degrees of PTBE, the lighter, the better—brain metastases can be get better control.5. Before the gamma knife radiotherapy,different degrees of PTBE(none, mild,moderate, and severe) have an impact on the prognosis of PTBE(p<0.05),This research shows that the heavier degree of preoperative PTBE, the worse be controled the degree of postoperative edema; On the other hand, he loghter degree of preoperative PTBE, the better be controled the degree of postoperative edema.6. The PTBE get local control by gamma knife radiotherapy, with a negative rate of96.2%; Out of control of PTBE with a negative rate of 69.2%. Patients, brain metastases under local control(effective) after Gamma Knife radiotherapy, whose the negative rate of PTBE is higher than the uncontrolled. It suggests the relationship between PTBE and tumor size, at least proves that the larger metastatic tumors more easily with heavier degree of peritumoral edema.Conclusion:The gamma knife radiotherapy for brain metastases and PTBE have higher control rate, and the size of brain metastases and the degree of PTBE affect the efficacy of gamma knife radiotherapy.
