Objective:To investigate the change in serum level of hypoxia inducible factor-1ɑ(HIF-1ɑ)and its clinical significance in congenital heart disease(CHD) complicated.Methods:Sisty-six case of CHD,including 16 without PH,18 with mild,17 with moderate,and15 with severe PH were recruited in the study.A contemporary cohort of 20 healthy children was selected as the healthy control group.All children underwent color Doppler ultrasonography for measurement of cardiac pulmonary artery systolic pressure(PASP),measuring blood oxygen pressure(Pa O2) by GEMPremier3000 blood gas analyzer.Test of Serum concentrations of HIF-1α using double antibody sandwich enzyme-linked immunosorbent assay(ABC-ELISA).Results:1.The results of serum HIF-1α in each group there was statistically significant difference between the 5 groups of children(F = 98.501, P <0.05).Comparison of the serum HIF-1α : None PH groups and normal control group showed no significant difference(P> 0.05); CHD groups were higher than the CHD without PH groups and normal control groups(P <0.05);mild PH groups were higher than the non-PH groups(P<0.05); moderate PH groups were higher than the mild PH groups(P <0.05); than and severe PH groups were higher than the moderate PH groups(P <0.05).2.The relationship between PO2 and PASP in each groups Pearson correlation analysis showed,PO2 was negative correlated with the PASP in each groups(r = 0.825,P <0.05).3.The correlation between the serum levels of HIF-1α and PO2 There was statistically significant difference between the 5 groups of children(F = 41.347, P <0.05).Pearson correlation analysis showed that the serum levels of HIF-1αand PO2 was negatively correlated each group of children(r =-0.552, P <0.05).4.The relationship between serum HIF-1αand PASP in CHD merger with PH Pearson correlation analysis showed, PASP was positively correlated with the serum level of HIF-1αin CHD merger PH groups(r = 0.825, P <0.05), with increasing of pulmonary artery systolic pressure, HIF-1αlevel increased.Conclusions:1. The study found that CHD complicated with PH, serum level of HIF-1α was increased significantly and positively correlated with pulmonary artery systolic pressure.The study suggest that HIF-1α was increased against cardio vascular injury caused by lack of oxygen, HIF-1α was related to the severity of pulmonary hypertension.The change in serum HIF-1α may somehow reflect the severity of pulmonary hypertension in CHD children.2. Serum level of HIF-1α may be used to monitor the development of CHD merger with PH. it may have clinical value.
