Objectives:Through the determination of content of nonionic contrast medium allergy and the control group in mast cell chymase, PAF and urine PGD2, to observe the changes of allergy in vivo chymase, PAF and PGD2 as biological indicators. To further explore the mechanism and significance of these indexes in contrast agent iodine allergy.Methods:20 experimental nonionic contrast agent allergy, aged 24~67, male 6 cases, female14 cases. 60 cases of beta lactam antibiotic allergy, aged 15~82, male 38 cases, female 22 cases. The control group of normal persons(excluding various immune allergic disease,coronary heart disease, brain injury, morphine overdose etc.) in 133 cases, aged 28~45,male 69 cases, female 64 cases. The groups are taking anticoagulant(EDTA) and non anticoagulant peripheral venous blood 2m L and urine 5m L, 1500 r/min, the centrifugal supernatant, 5 min after placement- 80 ℃ under test.(1) To determine the experimental groups chymotrypsin, PAF levels in serum by using the double antibody ELISA method.(2) Removing all urine samples at the same time at room temperature after melting and fully dissolved, ELISA method to detect the PGD2 in urine,and HITACHI 7170 A automatic biochemical analyzer to detect urinary, with results shown in urine PGD2 /urinary creatinine(ng/mg), to exclude the effects of renal excretion of factors.(3) To analyze and describe the data with statistical software.Results:(1)Nonionic contrast medium allergic group and beta lactam antibiotic allergy serum chymase content(1.03±1.79 ng/m L) is higher than the normal control group(0.08±0.03 ng/m L)(P < 0.05), but the two allergic groups(allergy 1.16±2.46 ng/m L)differ without statistical significance.(2)Serum PAF levels in nonionic contrast medium allergic group(22.67±32.80ng/m L) and beta lactam antibiotics Allergy Group(18.81 ± 29.08ng/m L) were higher than those in normal control group(1.14±0.83ng/m L)(P < 0.05), however,differences between two groups had no statistical significance.(3)Serum PGD2 levels in nonionic contrast medium allergic group(276.52±177.46ng/m L) and beta lactam antibiotics Allergy Group(261.56±160.22) were higher than those in normal control group(86.91 ± 31.35)(P < 0.05), however, differences between two groups had no statistical significance.(4)Radiography agent Allergy Group(55.72 ±60.04) and drug allergy group(56.03± 40.02) serum TNF- content was higher than that in normal control group(21.05 ± 8.51)(P=0.01), but the serum TNF- content of two groups differ without statistical significance.(5)Urine PGD2/urinary creatinine levels in non ionic contrast medium allergic group(28.33± 18.44) and beta lactam antibiotics in the allergic group(33.03 ± 25.54)were higher than the normal control group(6.31±5.24)(P < 0.05), however, differences beween two groups had no statistical significance.Conclusons:(1)As a specific marker of mast cells, mast cell chymase has become the biological indicators of mast cell degranulation, therefore, anaphylaxis, chymotrypsin is expected to be used as a biological index for allergy diagnosis.(2)Platelet activating factors were synthesized under the specific antigen receptor stimulation by the basophil activation, PAF combination with target PAF receptors on the cell membrane produce a large amount of active oxygen, leukotrienes and otherinflammatory mediators, play an important role in allergy, is also one of the important medium allergic reactions. It is expected to serve as biological markers to allergy diagnosis.(3)The macrophages produce TNF alpha tumor necrosis factor, which can promote the inflammatory cells aggregation and mediated hypersensitivity disease. The detection of TNF- alpha can be used as a marker to determine the existence of an allergic reaction.(4)The level of PGD2 in urine can be used as one of biological indexes in allergy diagnosis, but need to exclude the influence of renal excretion of factors.
