Objective:To compare the Inching technology with other nerve conduction studies(NCSs) in the diagnostic value of carpal tunnel syndrome(CTS).Methods:Consecutive 41 patients with CTS and 32 normal controls were enrolled to undergo inching technology of wrist median nerve.At the same time, the application of conventional method and comparative method to detect the wrist median nerve and of the same limb radial and ulnar nerve conduction.Results:(1) Comparison of the conventional method and the comparative method of the nerve conduction test in the the CTS group and control group: used the conventional method to detect median nerve across the wrist, the distal sensory latency and the distal motor latency(DML), and used the comparative method to detect the median nerve across the wrist and of the same limb ulnar or radial nerve sensory or motor nerve conduction latency difference in the CTS group were all longer than the control group, the difference was significant(P<0.01);The CTS group of the median nerve of the forearm movement conduction velocity slower than the control group, the difference was significant(P<0.01).(2) Inching technology test results comparison in the CTS group and the control group: In the CTS group of the segment from under the wrist 2 cm to the 3 cm below the wrist and the carpal tunnel section of the per centimeter of latency difference(LD/cm) were longer than the control group, the difference was significant(P<0.01).(3) The comparision of the sensitivity and specificity of the each NCS index in the CTS group:The Inching technology of the wrist median nerve to get the highest sensitivity, especially the detection sensitivity of the sensory Inching technology up 95.1%,compared with the conventional method and the comparative method, the difference was statistically significant(P<0.05); while in the motor nerve conduction using the conventional method and the comparative method to detect, 2LI-MDLD detection sensitivity(80.5%) was the highest.Conclusion:(1) 2LI-MDLD is the most valuable motor nerve conduction testing technology for diagnosis of CTS.(2) The Inching technology for CTS localization diagnosis with high sensitivity is superior to the conventional method and comparative method of nerve conduction testing technology that can be used as screening and diagnosis of CTS specific tools.
