Objective: Initial explore the relationship between H-hypertension and coronary lesion, and investigate whether the H-hypertension can be an independent risk factor for coronary heart disease.Methods:Select 129 hypertensive patients who was in suspected or confirmed coronary artery disease admitted to the First Affiliated Hospital of Shihezi University School of Medicine from December 2013 to Jun 2014. They all agreed to undergo the coronary angiography.1. All the patients were based on the level of homocysteine and divided into the H-hypertension group and the non-H-hypertension group. The two groups were compared their clinical data, while we were compare the percentage of the patient who was diagnosed with coronary heart disease in the H-hypertension group and non-H-hypertension group.2. Based on the CAG results, all the patients who were according to the coronary artery lesions involving the count were divided into single-vessel disease, double vessel disease, and multivessel disease group. We compared with the non-H-hypertensive group and the H-hypertensive group in each the percentage of the three groups.3. According to the result of the CAG, With Gensini integral method as the evaluation standard, determine coronary artery lesion severity, compare H hypertension with simple hypertension group Gensini score,at the same time to explore the correlation between Hcy and Gensini score. 4.According to CAG result,excluding non-coronary heart disease in patients, select the patient with acute coronary syndrome,comparison of patients with acute coronary syndrome in combination with the H-hypertension with CHD and the non-H-hypertensive with CHD. 5. CHD risk factor screening method is non condition Binary Logistic regression analysis. After data collection is good, the application SPSS18.0 statistical software to establish a database and analyzed. First and homogeneity of variance normality, qualified measurement data using mean ± standard deviation, t test was used to compare the two groups using analysis of variance.Count data using chi-square test. Pearson correlation analysis of two continuous variables. Logistic regression analysis of risk factors for coronary heart disease screening, Inspection standards for α = 0.05, P<0.05 was considered statistically significant.Result:(1)88 H-hypertensive patients and 41 non-H-hypertensive patients. Sex ratio of the two groups,smoking history, history of diabetes, age, triglycerides, total cholesterol levels, LDL, HDL levels were compared, the results suggest two groups is balanced comparable. The CHD prevalence of H-hypertension is 82.9%, higher than the prevalence of coronary heart disease in non-H-hypertensive group is 65.9%, the difference was statistically significant(P<0.05).(2)The proportion of patients with coronary artery disease among H-hypertensive branched array pairwise comparison found that among single vessel disease group and double-vessel group, among single-vessel disease and multivessel disease group difference was statistically significant(P <0.05); and in between the double-vessel disease and multivessel disease group, the difference was not statistically significant(P> 0.05).(3)In the patients with coronary heart disease, H-hypertensive patients with Gensini score higher than the average non-H-hypertension group, the difference was statistically significant(P<0.05).(4)The Gensini score and coronary heart disease in patients with Hcy levels were positive linear correlation(r=0.312, P<0.01), it shows that the Gensini score with increasing levels of Hcy rises.(5)H-hypertension and coronary heart disease in combination with non-H-combination hypertension and coronary heart disease compared with patients, the incidence of acute coronary syndrome is more likely,the difference was statistically significant(P<0.05).(6)With a history of smoking, H-hypertension, homocysteine, diabetes, triglycerides, total cholesterol,low density lipoprotein, high density lipoprotein as independent variables to whether coronary heart disease as the dependent variable, the use of binary linear Logistic regression analysis. Which triglycerides, total cholesterol, low density lipoprotein, high density lipoprotein, homocysteine for continuous variables,directly into the equation. H-hypertension, smoking, diabetes, history as a categorical variable, later converted into dummy variables included in the equation. Independent risk factor analysis showed H-hypertension, homocysteine, smoking history, history of diabetes are coronary heart disease.Conclusions:(1)H-hypertension is closely related to the severity of coronary artery disease;(2)H-hypertension patients have easier to get acute coronary syndrome;(3)H-hypertension is an independent risk factor for coronary heart disease.
