Objective 1、By measuring femoral artery intima-media thickness(FA- IMT) and ankle arm index(ABI) in order to analyze the correlation between FA-IMT、ABI and coronary heart disease and evaluate predictive value of the degree of coronary artery stenosis in patients with CHD.2、Analysis the correlation between ABI and femoral artery IMT.MethodsThere were 98 patients who underwent coronary angiography were collected from the first hospital affiliated to medicine institute of Shihezi university during December 2013 to July 2014. According to the results of radiography they were divided into four groups: There were 31 cases in one-vessel stenosis group. Male: 22 cases. Female: 9 cases, the average age is(58±8) years. There were 27 cases in two-vessel stenosis goup. Male:18 cases. Female: 9 cases, the average age is(58±10) years. There were 16 cases in three-vessel stenosis group. Male: 11 cases. Female: 5 cases, the average age is(62±9) years. There were 24 cases in healthy controls group(normal group). Male: 13 cases. Female: 11 cases, the average age is(57±6) years.All patients were involved in the study with agreement. All chosen people undertook femoral artery ultrasound, ABI test, history-taking and routine laboratory tests before operation. Analyzed the healthy controls group with different levels of coronary artery lesions groups, whether the difference between FA-IMT and ABI of which was statistical significance. To analyze the correlation between FA-IMT、ABI and coronary heart disease and evaluate predictive value of the degree of coronary artery stenosis in patients with CHD.Results1. Compared Sex, age and history of cerebral apoplexy among groups(P>0.05), there was no obvious statistical significance. Compared Body mass index,history of hypertension and smoking history among the four groups(P<0.05), they had difference and there was statistical significance.2. Compared total cholesterol(TC), high-density lipoprotein(HDL) and Lipoprotein a(LPa) among the four groups,they had no significant difference and there was no statistical significance(P > 0.05).Compared low-density lipoproteins(LDL), triglyceride(TG) and homocysteine(HCy) among the four groups, they had no difference and there was statistical significance(P>0.05).3.There was obvious statistical significance in FA-IMT compared among the four groups. and it was statistical significance(x2=41.35,P<0.01). The results of FA-IMT in the case group [one-vessel stenosis group(0.17±0.07)cm,two-vessel stenosis group(0.24±0.07) cm,three-vessel stenosis group(0.34±0.06)cm]was significantly higher than the healthy controls group [(0.15±0.10)cm],P<0.01.There was obvious statistical significance in ABI compared among the four groups(x2=21.90,P<0.01).The results of ABI in the case group [one-vessel stenosis group(0.98±0.18),two-vessel stenosis group(0.86±0.14),three-vessel stenosis group(0.86±0.13)]was significantly lower than the healthy controls group[(1.06±0.17)], P<0.01.4. Interrelated analyses conclude that:FA- IMT prediction of coronary atherosclerotic: The sensitivity was 75.7%,the specificity was 50%,the positive predictive value was 82.4%,the negative predictive value was 40 %,the positive likelihood ratio was 1.51,the negative likelihood ratio was 0.49;The ABI prediction of coronary atherosclerosis: The sensitivity was 68.9%,the specificity was 70.8%,the positive predictive value was 87.9%,the negative predictive value was 42.5 %,the positive likelihood ratio was 2.36,the negative likelihood ratio was 0.44.The FA – IMT and the ABI predictive value of coronary atherosclerosis was nice.5. Spearman rank correlation analysis showed that the FA- IMT and coronary atherosclerosis were positively related, the correlation coefficient R = 0.649;ABI are negatively related with coronary atherosclerosis, correlation coefficient R = 0.459; there was a negative correlation between FA-IMT and ABI, and the correlation coefficient R = 0.606.Conclusion 1.Body mass index,history of hypertension,smoking history,ascending low-density lipoproteins L(LD),ascending triglyceride(TG) and homocysteine(HCy) are risk factors of coronary atherosclerotic heart disease.2.FA- IMT and ABI can predict the degree of coronary artery stenosis in patients with coronary artery disease.3. The thicker the FA-IMT, the hint of coronary heart disease may be narrowed coronary arteries, the lower ABI, prompt the more serious the coronary heart disease may be narrowed coronary arteries, the joint diagnosis of coronary heart disease(CHD) effect is better.
