Purpose:To observe the expression of TNF in blood serum and pleural effusion, and the pleural tissue ultrastructure of rats with pleural effusion using the prescription (Ling Gui Zhu Gan Tang added up to Ting Li Da Zao Xie Fei Tang) which represented the therapy of yang-warming to resolve fluid retention,and to investigate some of the mechanism of treating the fluid retention by the therapy of warming yang to resolve fluid retention on the molecnlar level.Methodology:40 of healthy adult wistar rats (male and female half), adaptive feed after a week, weight 200-250g, random grouping them by computer, divide into four groups, the yang-warming and fluid retention-resolving therapy group (W), wiping off the cassia twig fluid retention-resolving group(Q), model control group(M) and bank control group (K), each group with ten rats. All rats expect those in blank control group received intrapleural injection of 1% carrageenan to establish a rat model of pleural effusion. At 24 hours after the model was established, model rats were subjected to pathological evaluation by radiography. At 2h after the model was established, the W and Q group received respective treatments by gavage once daily for 3 d. All rats were sacrificed after intragastrically administration, collecting the blood serum, pleural effusion and pleura. Using enzme-linked immune sorbent assay(ELISA) determination to measuring the content of TNF-α in the blood serum and pleural effusion. Using the electron microscope to observe the organizational structure about pleura, and the results were compared between each group.Results:(1) The expression of TNF-α in the blood serum: ① compared with the blank control group,the TNF-α in the model group is increased (P<0.05),there is no significant difference between the W group and Q group (P>0.05);② the expression of TNF-α are all decreased in the W group and Q group contrast with the model group (P<0.05);③ there is no difference between the Q and W group (P>0.05). (2) The expression of TNF-α in the pleural effusion: ① compared with the model control group, the expression of TNF-α in the Q and W group are all decreased (P<0.05);② there is no significant difference between the Qgroup and W group (P>0.05). (3) Pearson correlation analysis about TNF-α between the blood serum and pleural effusion:there is a positive correlation about the expression of TNF-α in the M group (r=0.573,P< 0.05); the concentration of TNF-α in the Q and W group has no correlation between the blood serum and pleural effusion(P>0.05). (4) The consequence of pleura by transmission electron microscope:the visceral and wall of pleura are the same, there are sparse micro villi on the surface of mesothelial cells and the collagenous fiber are few and far between in the M group;there are rich microvilli on the surface of mesothelial cells, collagenous fiber and closely packed in the blank group,M and W group.Conclusion:(1) Carrageenan to establish a rat model of pleural effusion can lead the rat to produce pleural effusion and rise the expression of TNF in blood serum and Pleural effusion of the rats.(2) The substitute decoction of the yang-warming and fluid retention-resolving therapy about Ling Gui Zhu Gan Tang added up to Ting Li Da Zao Xie Fei Tang and Ling Gui Zhu Gan Tang added up to Ting Li Da Zao Xie Fei Tang wiping off the cassia twig can decreased the expression of TNF-α in the blood serum and Pleural effusion, reliving inflammation. It may has the correlation with the function of soil water making by the decoction to eliminating the pleural effusion.(3) There are no significant difference in decreasing the expression of TNF-α in the blood serum and pleural effusion between the yang-warming and fluid retention-resolving decoction and wiping off the cassia twig fluid retention-resolving decoction.(4) The yang-warming and fluid retention-resolving decoction and wiping off the cassia twig fluid retention-resolving decoction is benefit for renewing the content of microvilli on the surface of Pleura mesothelial cells, the collagenous fiber are closely packed about the model rat, It may be another function to eliminating the pleural effusion by the two decoctions.
