Objective: to screen the differentially expressed proteins in the placenta of preeclampsia, and to study the correlation between the protein and the adverse pregnancy outcome. Methods: using pathway protein chip technology(protein pathway array, PPA) for 64 phosphorylated or nonphosphorylated antibodies, measured in 101 cases of preeclampsia and 65 cases of control group of pregnant women placental protein expression. Sam used statistical software found preeclampsia group and the control group have the difference of expression of proteins, The study on protein parameters were analyzed, application SPSS17.0 take P < 0.05 were considered to be statistically significant. Screened proteins related with preeclampsia adverse outcomes.Result:(1) Clinical features:The pathogenesis of preeclampsia, and maternal fetal outcomes of diagnosis and other relevant data in Table 4.1 summarizes, in maternal age, gestational age, birth gestational age, platelet count, birth weight factors, and compared with the control group of preeclampsia group was significantly lower; and maternal body mass index, maximum diastolic blood pressure, the highest the systolic blood pressure, uric acid, creatinine and ALT, the control group was significantly higher than.101 patients with preeclampsia in 81 cases of severe preeclampsia, the other 17 cases of 48 cases of early onset preeclampsia in mild.101 patients with preeclampsia, 53 cases of late onset in patients with preeclampsia has no adverse outcomes show in table 4.2 to table 4.4. pregnant women with gestational age less than 34 weeks. Gestational age, gestational age, platelet count, birth weight was significantly lower than that of no bad end group; while the highest systolic blood pressure, the highest diastolic blood pressure, ALT, uric acid, creatinine was significantly higher than that of the no adverse outcome group; the age and body mass index of pregnant women were the same in those with no adverse outcomes.(2) Preeclampsia group and control group of differentially expressed proteins:(P<0.0002 classifier(K=10 cross expression was detected in 64 protein and phosphorylation of protein, the difference of 36. Sam statistics software analysis, in which 36 protein and phosphorylation of protein 19 is up-regulated,(including calretinin, flt-1 Htr A). While the other 17 is down regulated(including Annexin XI).(3) To assess the adverse outcome of preeclampsia in the associated proteins:Analysis of detection of 64 kinds of protein and phosphorylation of protein, the application of Sam(q<0.05, false discovery rate of less than 5%) obtained three proteins including Flt-1, endoglin and PAI-1 in preeclampsia with poor outcome group is positively regulates the expression. In order to confirm the relationship between the protein and the pathogenesis of preeclampsia adverse outcome, single factor regression analysis and multivariate regression analysis were applied to Flt-1, endoglin and PAI-1. These three proteins is still as pre eclampsia occurred independent risk factors for adverse outcomes.Conclusion:(1) There were 19 differentally expressed proteins in preeclampsia group and control group, but the specific mechanism of action of each protein in the occurrence and development of the disease need to be further studied.(2) Application of protein chip technology to screen out of 36 differentally expressed proteins, and through the further statistical analysis found that PAI- 1 and FLT 1 and Endoglin is associated with aderse outcome in preeclampsia occurs and may serve as potential forecasting preeclampsia consolidation potential biological indicator of adverse outcome(3) This study found that the highest diastolic blood pressure in pregnant women, yun ling, creatinine is independent risk factors for preeclampsia and adverse outcome, thus can be considered as a reference index to evaluate the degree of disease.
