Continuous Hemodialysis Filtration In Multiple Organ Dysfunction Syndrome In The Role Of Inflammatory Mediators

Objective: Discuss continuous hemodialysis filtration serum inflammatory mediators clearance and prognosis in patients with MODS.Methods : select the fifth affiliated hospital, xinjiang medical university in September 2013- December 2015 in MODS or SIRS or sepsis in 40 patients with any kind of diagnostic criteria, cases were divided into treatment group and 22 cases in the control group.Patients in the control group routine medical treatment, the treatment group patients to undergo medical treatment + CVVHDF continuous treatment for more than three days.Analysis of 3 days in the two groups before treatment patients with inflammatory cytokines in serum concentration changes, using ELISA method for determination of serum TNF-α, IL- 6, IL- 10 level;Closely observe and record the patient vital signs, BUN and Cr.At the same time the total hospitalization days and gave birth to village rate were observed.Results : 16 cases of treatment group survival, survival rate is 72.73%;Control group 6 cases, survival rates were 38.89%, the survival rate is higher than the treatment group, treatment group survival rate comparison of two groups, the difference was statistically significant(P < 0.05).Treatment group total length of hospital stay was14.60 ±4.12 days,control group total length of hospital stay was18.20 ±4.51 days,the treatment group the total length of hospital stay was shorter,the total length of hospital stay compared the two groups,the difference was statistically significant(P < 0.05).Treatment group aftertreatment TNF-α,IL- 6, IL- 10 in serum concentration,APACHE II score, BUN and Cr significantly lower than the control group,the difference was statistically significant(P < 0.05).Treatment group after treatment, heart rate, mean a rterial pressure,oxygen saturation obviously improved than the control group after treatme-nt, the difference was statistically significant(P < 0.05). Conclusion: CVVHDF canremove MODS patients with inflammatory mediators in the body, regulate the body’s homeostasis,to help restore the body’s immune function.To improve the clinical symptoms and prognosis.
