The Research Of The Lateral Nasal Artery Anatomy And Clinical Application

ObjectiveA probe and discussion about the move towards and distribution of lateral nasal artery(LNA),and to provide anatomic basis for clinical application.Methods Drawing 40 patients form Department of Plastic Surgery, The Affiliated Provincial Hospitial of Anhui Medical University, detected the orientation of the LNA under the doppler ultrasound instrument to make data analysis,and 5 cases intraoperative vivisection for LNA. the aesthetic effect of the operation is determined according to doctors, patients and their families satisfaction.Results 5 cases intraoperative vivisection of LNA, no one is absent. Selected 40 patients, applying doppler ultrasound instrument to locate the LNA on alar groove edge midpoint, the date distribute between 5.00-8.00 mm, the average 6.50 mm, the variance 0.65 mm,the range of 2.00～3.00 mm.40 pairs symmetry distribution of LNA, which are 80 LNA, none is absent.12 patients with defects on the tip of the nose, were treated using island flap which nourished form LNA,got one-stage repair. Average flap diameter (long axis) 16 mm, minimum diameter 9 mm, maximum flap for 22 x 36 mm. Follow-up was done for 3 to 12 months,all the flap survived,and all wound got one-stage healing. By comparing the preoperative surgery photograph with the postoperative photograph to estimate the operation effect,10 cases of surgery effect are good (83.33%),2 cases of surgery effect are general (16.67%),0 case of surgery effect is poor (0%), and surgery gets well aesthetic effect.Conclusion Benefit by the stablely anatomy of LNA,the one-stage repairation of the defects on the tip of the nose with island flap which nourished form LNA,is a practical and reliable choice.The innovation of this study:① Application of the doppler ultrasound instrument to detect the distribution of LNA and data analysis, has a good guiding significance for clinical practice.② This flap is designed to repair the nasal tip nose small skin and soft tissue defect the on a third part under nasal, its characteristic is that for disc area is located in the lateral nasal back, no matter from the skin texture and color sebaceous skin pore structure, this flap is more close to the tip of nose,and good aesthetic form. For the area is relatively hidden, without secondary iatrogenic deformity legacy; A period of repair and reduce secondary surgery in patients with painThe deficiency of this study:① Doppler ultrasound instrument posite the across of the artery and the lateral nasal blood vessel and make data analysis, but its cannot reach the accuracy of autopsy and angiographic, data template is small, it need to be further improved.② Outer blood vessels in anatomy, limitations, only shows that the existence of the blood vessels, could not confirm its source of blood vessels.③ This study of 12 patients with preoperative photos comparing to postoperative photos, performer and patients were satisfied with the nasal aesthetic results after repair. But because of the less number of cases, we can not make aesthetics of statistics analysis, subject to further clinical accumulation.
