Objective:To investigate the risk factors whichlead to the rapid progression o f renal function of lupus nephritis (LN) patients, to describe the patient’s gene ral situation, and to analyze the correlation between laboratory parameters a nd rapid progression of renal function, finally, to provide epidemiological evid ence for clinical treatment and improvement of prognosis.Methods:Perform retrospective analysis of 1607 LN patients during Januar y 2009 to September 2014 from Department of Renal and Department of Rh eumatism,the First Affiliated Hospital of Anhui Medical University, of which 175 cases are with rapid deterioration of renal function. Describe the age, edu cational level, residence, marital status and treatment compliance of the patient s. Randomly select 108 cases without renal impairment as control group. The ra pid progression of renal function is defined as dependent variable, age, recent i nfection, hypertension, electrolyte disturbance, lupus activity, cardiac insufficienc y, white blood cells, anti histone antibody (AHA) positive, anti nucleosome anti bodies (ACA) positive, complement C3 decreased clinical data index are define d as independent variables. Perform logistic regression analysis to explore the r elated factors and deterioration factors.Results:Among patients with rapid progression of lupus nephritis renal function, The majority were women of childbearing age with a history of multiple preg nancy, and often with lower education and poor compliance fromrural populatio n, The SLEDAI score for Infection, severe anemia, low protein hyperlipidemia, SLEDAI score is more than 10. And hypertriglyceridemia is the most significan t factor (P< 0.05)of rapid progression of renal function.Infection, hypoalbumi nemia, lupus moderate activity are the also risk factors for rapid deterioration o f renal function(P< 0.05).Conclusion:It is important to strengthen propaganda and education of the patien ts and their families, and to improve their understanding of the disease. Measur es should be taken to look for incentives of LN patients who had recent renal function impairment. In order to improve renal function,immunotherapy for LN patients should be adopted together with active infection control and nutritional support treatment.
