| Objective:Periarthritis of shoulder, also known as around shoulder joint inflammation, commonly known as coagulation shoulder, frozen shoulder. The shoulder joint surrounding tissues get degenerative degeneration without a clear cause. The cold attacks, chronic and the effect of factors such as strain, sprains, can cause aseptic inflammation around the shoulder joint and gradually result in adhesion to shoulder joint dysfunction. The pain of shoulder is an incipient symptoms with serious at night and increase gradually making the shoulder joint activity function limitation happen gradually. Most of patients are manual workers at the age of 50 years and female incidence is slightly higher than male. If they can not get effective treatment, the function of shoulder joint activity may be seriously affected, and the different degree of deltoid muscle atrophy appeared. The common treatment for periarthritis of shoulder contain drug therapy, pain points and local closed therapy, physical therapy, traditional massage therapy, and exercise therapy, etc. But the treatment effect is not exact with slow effect, long treatment cycle。In recent years, with the development of surgical operation, arthroscopic shoulder, needle knife and other treatment methods are emerging, but the high cost of treatment and belongs to the invasive treatment, limits the scope of its application in clinical. Extracorporeal shock wave (Extracorporeal shock wave, ESW) is a kind of mechanical pulse pressure wave transmit through the physics mechanism conduction medium.Its first application was the treatment of urinary stones and now has been widely applied in the treatment of bone nonunion, delayed fracture healing and diseases such as acute or chronic soft tissue injuries which has achieved remarkable curative effect. Extracorporeal shock wave treatment has good treatment effect by controlling the movement and positioning of probe for a large area of human tissue pain and discomfort. This study through the application of extracorporeal shock wave with inflammation around shoulder joint function rehabilitation exercise treatment, observation of patients around the shoulder joint inflammation relief and recovery, to explore the role of too shock wave treatment around the shoulder joint inflammation, not regulatory status will provide new options around phlogistic treatment. This study applies extracorporeal shock wave of inflammation around shoulder joint function combined with rehabilitation exercise treatment, observes the relief of shoulder joint inflammation and recovery, to explore the role of shock wave treatment around the shoulder joint inflammation.Methods:This experimental is designed as a single blind randomized controlled trial.90 patients were selected from March 2014 to January 2014 in our department of clinic treatment with shoulder problem.60 patients were token into the experiment who conform to the standard of the experiment. They were randomly divided into Extracorporeal shock wave treatment group (Extracorporeal shock wave, ESW) and control group (n=30). Patients with around the shoulder joint inflammation of two groups were given basic functional rehabilitation exercise therapy, including Chinese medicine massage therapy, loose shoulder joint technology and exercise therapy. At the same time, extracorporeal shock wave (ESW) treatment group was treated in basic function rehabilitation exercise besides accept conventional shock pressure of 2.0-3.08 bar,5 -- 10 Hz frequency, impact times,3000 times of extracorporeal shock wave therapy, once a week for 4 weeks, a total of treatment 4 times. Assessment was taken at the start and the end of the treatment and at the follow-up 4 weeks after treatment by the method of visual analogue scale (visual analogue scale, VAS) for assessment of shoulder pain in patients and using the function recovery of shoulder joint function assessment scale evaluation of the shoulderResults:The compliance of the patients is well and all of he 60 patients have completed the experiment. There were not obvious side reactions such as skin allergy, swelling and spasm in the process of the experiment.1.The basic data of the selected patients such as age, sex and the course of the disease have no statistical significant difference.2.The VAS scores of the patients with shoulder pain is significantly decreased after accept the treatment lweek and the difference is statistically significant(p<0.05) as well as the result of 4 weeks after the treatment and After 4 weeks follow-up.3.The evaluation of shoulder joint function between the two groups of patients improved significantly after the treatment and at 4 weeks later follow-up than before the treatment (p<0.05), the difference is statistically significant;Comparison between groups shows ESW is better than control group at various time points of evaluation results, p<0.05, the difference is statistically significant. |
