Objective:To investigate the relationship between secreted frizzled related protein 5(Sfrp5) with type 2 diabetes mellitus (T2DM) with macroangiopathy. To observe the effect of calcium dobesilate intervention on the T2DM with macroangiopathy. To investigate the role of calcium dobesilate in the improvement of T2DM macroangiopathy.Methods:(1)Selected 95 pitients with type 2 diabetes from march 2015 to july 2015, endocrine out-patient clinic and the inpatient department, Jingzhou hospital affiliated tongji medical college of huazhong university of science and technology (in 1999 WHO diabetes diagnosis and classification standard) for the experimental group. According to carotid intima-media thickness (IMT) examination,95 T2DM patients were divided into xperimental group (IMT≥ 0.90mm) 65 cases and control group (IMT<0.90mm) 30 cases. Other 20 normal persons were as normal group. IMT diagnosis according to the Guidelines for Prevention and Treatment of Hypertension in China 2010. IMT≥ 0.9mm value as endarterium thickened, IMT≥1.3mm value as atheromatous plaque, IMT< 0.9mm value as normal endarterium. People were observed for Sfrp5、IL-10、TC、 TG、LDL、FPG、2hPG、HbA1c、IMT.Statistical comparisons were performed by single-factor analysis of variance and Linear correlation analysis.(2) Selected 65 paitients with type 2 diabetic macroangiopathy from march 2015 to july 2015.Self-control and inter-group control prospective study was conducted in 65 cases of patients with diabetic macroangiopathy who were randomly divided into two groups after 16-weeks long treatments of calcium dobesilate (30cases) and blank(35cases, Only hypoglycemic therapy).Two groups of macroangiopathy patients had normal blood glucose control (Fasting blood glucose<7.0mmol/1,2-hour postprandial blood glucose<11.1mmol/1). The treatment is 16 weeks. Using paired t test,patients before and after treatment were observed in serum Sfrp5、 IL-10、TC、TG、LDL-C、FPG、2hPG、HbAlc、IMT.Results:(1) The levels of Sfrp5 and IL-10 in xperimental group and control group were significantly lower than those of normal group (P<0.01).These two indicaters in xperimental group were lower than control group (P<0.05).Correlation analysis showed that the levels of sfrp5 were negatively correlated with FPG 2hPG、TC、TG、LDL、HBAlc、the courses of T2DM、 BMI、IMT(r=-0.543、0.513、-0.454、-0.339、-0.468、-0.623、-0.576、-0.651、-0.56), the levels of IL-10 were negatively correlated with these indicates (r= -0.304、-0.209、-0.353、-0.275、-0.354、-0.410、-0.311、-0.506、-0.263).Sfrp5 was positively correlated with IL-10,r=0.502 (P<0.05).(2) After 16 weeks treatments of calcium dobesilate, the content of serum TC、TG、LDL、FPG、hPG、HbAlc and IMT in group A were decreased significantly compared to previous, and Sfrp5、IL-10 were increased signlificantly (P<0.01). B group were not given calcium dobesilate therapy,after 16 weeks,the content of TC、TG、LDL、FPG、2hPG.HbAlc and IMT in group B were decreased significantly compared to previous(P<0.01), Sfrp5 and IL-10 did not alter signlificantly, the difference were not statistically significant(P>0.05). IMT in A group were decreased significantly compared to B(P<0.05).Conclusions:The Sfrp5 and IL-10 play an significant role in the development in diabetic macroangiopathy, when the serum Sfrp5 and IL-10 concentration decreased promoted the occurrence and development of diabetic macroangiopathy.Given treatment of calcium dobesilate for patients with diabetic macroangiopathy could elevate the concentration of serum Sfrp5、IL-10, thus delay the progression of T2DM macroangiopathy.
