The Clinical Research For The Treatment Of Senile Vascular Demen Through Da Ming Decoction

ObjectiveThis study explains the mechanism of vascular dementia (VD) in traditional Chinese medicine.from the angle of“life-gate theory”. To observe the clinical curative effect of Da Ming Decoction(DMD) and nimoldipine in treatment of VD with clinical randomized controlled research, exploring the feasibility treatment of the law of "warm and tonify the life-gate" for the treatment of VD and providing scientific basis for clinical efficacy of Da Ming Decoction.Methods1. In this paper, to diagnose VD according to Western diagn ostic criteria the DSM-IV and the NINDS-AIREN diagnostic criteria, and the "Diagnosis, syndrome differentiation and therapeutic effe ct evaluation criteria of senile dementia" was used as the TCM diagnostic criteria for VD. All patients using the MMSE, HDS, A DL, TCM syndrome score for scoring, during the early, mid, or late treatment phase.2. Experimental group treat with DMT, whereas control group treated with nimoldipine, used 3months, one month follow-up meeting.3. Observation two groups before and after treatment, cognitive functions, and symptoms related to Chinese medicine tongue, pulse changes, drug use before and after a monthly record of regularly completing clinical Observation table. And recorded the adverse reactions may occur during treatment records at any time.4. Statistical treatment:all data using software SPSS-20.0 statistical analysis, measurement data showed a normal and homogeneity of variance using t test, variance not neat using corrected t test. At the same time of were detected before and after treatment, calculate the difference of the index before and after treatment, and the difference of between groups independent samples t test, measurement data before and after the treatment, its control by the paired t-test, count data using x2 test, using the Fisher’s exact probability when the cell or T<1 n<40, two groups compared by rank sum test.ResultsThe treatment of vascular dementia (VD) in 50 cases, including 2 cases of loss. (1) treatment group (23 cases) and MMSE score effect:4 cases were markedly effective,15 cases were effective,4 cases ineffective, efficiency of 82.6%;TCM curative effect score:4 cases were markedly effective, effective in 19 cases, invalid 0 cases, efficiency was 100%(2) control group (25 cases) and MMSE score effect:2 cases were markedly effective, effective in 12 cases, 7 cases had no effect, efficiency 72.0%; TCM curative effect score:0 cases markedly effective, effective in 18 cases,7 cases were invalid, efficiency of 72%. Curative effect before and after treatment in the two groups have significant differences were statistically significant (P＜0.05) (3) before and after the treatment of both MMSE scores had significant difference (P＜ 0.05), between the two groups MMSE score difference by statistical difference was statistically significant (P＜0.05), suggesting that two groups can improve can improve the score of MMSE, the treatment group than the control group; both before and after the treatment of HDS scores were significant difference (P＜0.05), the two groups before and after treatment score difference is significant difference was statistically significant (P＜0.05), suggest that the two groups can improve the HDS, the treatment group was better than that of the control group. The curative effect of TCM Syndrome by statistical treatment have significant difference (P＜0.05) before and after treatment in the two groups, between the two groups in the score difference compared with significant differences were statistically significant (P＜ 0.05), suggesting that both of them can improve the syndromes of traditional Chinese medicine, the treatment group was better than that of the control group.ConclusionsThe observation on Da Ming Decoction, showed that it has positive and safe effect in treating VD, Which can improve the patients’cognitive function and ameliorate the related TCM Syndrome. Which showed that treatment group is superior to the control group.All above indicated that DMD is one of the effective medicine in treating VD, and the law of "warm and tonify the life-gate" based on the life-gate theory for the treatment of VD is very feasible. Which worthy to be popularized in clinic.
