Objective:To analyze the therapeutic effect of endoscope and laparoscope in treatment of choledocholithiasisMethods:319 cases of choledocholithiasis patients were collected in our hospital from January 2010 to December 2015,all patients were divided into the ERCP combining EST group (ERCP+EST group, n=188) and Laparoscopic Common Bile Duct Exploration group (LCBDE group, n=131). The clinical data, effect of operations and postoperative complications of both two groups were analyzed.Results:The successful rates of operation of ERCP+EST group and LCBDE group were 93.6% and 87.0%, respectively, there were statistically differences (P<0.05). The hospitalization time, operation time, and exhaust time were significantly differences (P<0.05) between two groups; Rates of conversion to laparotomy during operation of both groups were 4.7% and 13.0%(P<0.05). In LCBDE group, diameters of common bile duct were> 10mm, the maximum diameter of calculi was>20 mm, and cases of calculi locating at the upper end of common bile duct were more than ERCP+EST group. Number of calculus, incidence rates of postoperative complications and postoperative fever were not significantly differences (P>0.05).The liver function were improved popstoperation in the two groups. and the indexes of ERCP+EST group were better than that the LCBDE group. Average hospitalization costs in LCBDE group were more than that the ERCP+EST group.Conclusion:1. ERCP combined with EST in the treatment of the choledocholithiasis is significant, particularly for the elderly patients in poor health.2.LCBDE was mainly applicable to the patients whose big choledocholithiasis was diffic ult to be taken out by the endoscope and with the gallbladder stones combined with choledocholithiasis.
